Previously, patient PNP (49 years old, residing in Hoa Vang District, Da Nang City) was admitted to Da Nang Hospital with a complicated wound, his left hand was cut off with a chainsaw.
Specifically, the patient had his first finger (thumb) and third finger (middle finger) severed, the remaining fingers were damaged. Of the two severed fingers, the thumb was lost at the scene, only the middle finger was taken to the emergency room.
Immediately, the doctors on duty at the Department of Trauma and Orthopedics decided to reattach the severed finger using microsurgery for the patient.
The patient's thumb was reattached from the previously severed middle finger.
After nearly 5 hours, doctors successfully reattached the middle finger to the thumb and treated the damage to the remaining fingers.
The surgical team said that the decision to attach the middle finger to the thumb was because the thumb accounts for 50% of the hand's function. In the opposite position, the thumb combines with the remaining 4 fingers to form a pincer to grasp and pick up objects.
Therefore, injuries that cause loss of the thumb will reduce hand function, seriously affecting the patient's work and daily life, so thumb reconstruction is very important.
According to doctors, the difficulty of this case is the complicated hand injury. In particular, the arteries and veins of the severed finger were crushed for quite a long time, along with the incompatibility in the size of the blood vessels of the two anastomoses, leading to difficulty in connecting the blood vessels and a very high risk of vascular occlusion. Therefore, the surgery time is long, requiring perseverance and meticulousness.
Up to now, the patient's thumb has been able to move and the patient has been discharged from the hospital.
Da Nang Hospital recommends that severed limbs should be preserved by wrapping them tightly with gauze or clean cloth (not too thick) around the severed part, then placing them in a thin plastic bag, tying the bag tightly so that water cannot seep in. Place the bag in an ice chest or a basin of ice and quickly bring it with the patient for timely emergency treatment.
