Dangerous complications of obese patients

Obesity is a major risk factor for heart failure and sudden death due to its association with coronary artery disease, diabetes, hypertension, and dyslipidemia.

Three days before being hospitalized, Mr. Chan (42 years old, from Dong Nai) felt tired, had chest heaviness, epigastric pain, and nausea after eating. He went to a local medical facility for examination and was diagnosed with gastric reflux and given medication. The condition did not improve, fatigue and difficulty breathing increased, and he vomited everything he ate, so his family took him to the emergency room.

Overweight and obesity have become a global problem, a global pandemic.

The patient was admitted to the hospital in a state of acute heart failure: fatigue, sweating, sitting with retracted breathing, rapid pulse, little urine, clamped blood pressure (the gap between systolic and diastolic blood pressure is less than or equal to 20 mmHg) with pulse indexes of 120-130 beats/minute, blood pressure of 100-110/70-80 mmHg.

Chest X-ray results showed acute pulmonary edema, bedside echocardiography confirmed large dilated heart chambers, significantly reduced left ventricular contractility (EF = 10-15%). Electrocardiogram showed signs of acute myocardial infarction in the anterior-lateral apex region with elevated cardiac enzymes, renal failure and elevated liver enzymes. The doctor diagnosed acute myocardial infarction with complications of multiple organ damage, dilated cardiomyopathy, grade 3 obesity (110 kg, BMI = 40.75).

Mr. Chan was treated with intensive resuscitation to treat acute pulmonary edema, then transferred to the DSA room for coronary angiography and emergency reperfusion intervention. The results showed that the anterior interventricular artery was completely blocked with multiple blood clots. The interventional team performed angioplasty and placed a stent at the blocked coronary artery.

Doctor Nguyen assessed that the patient's clinical condition was very serious, with a high risk of death due to severe heart failure, large dilated heart chambers, and multiple organ failure. He urgently needed angioplasty and stent placement at the blocked coronary artery.

MSc. Dr. Vo Anh Minh, Deputy Head of Interventional Cardiology Department, Cardiovascular Center, Tam Anh General Hospital, Ho Chi Minh City, said that the patient had severe heart failure due to complications of dilated cardiomyopathy and acute myocardial infarction, with a very high risk of arrhythmia and cardiac arrest during angioplasty.

The doctor decided to run ECMO (artificial heart-lung system), placing a counter-pulse balloon before and during the procedure to maintain blood pressure, filter blood, and reduce the risk of cardiac arrest on the operating table.

Chan's dilated cardiomyopathy was the result of long-term untreated obesity. Chan had a sedentary lifestyle, so the symptoms of heart failure were not obvious. In recent years, he occasionally felt tired and out of breath when doing heavy work, but he thought it was because he was fat, so he did not go to the doctor.

If an acute myocardial infarction does not occur and the patient does not go to the hospital, the undetected dilated cardiomyopathy will continue to progress, causing many dangerous complications such as heart failure, arrhythmia and sudden death, Dr. Huy affirmed.

To reduce the risk of cardiovascular and metabolic diseases, overweight and obese people should go to medical facilities to get advice on appropriate treatment regimens and try to bring their weight back to normal limits.

Patients must have their cardiovascular function assessed, blood pressure, blood sugar, and blood lipids checked periodically. When abnormalities such as high blood pressure, diabetes, heart dysfunction, etc. are detected, treatment is needed immediately even if there are no symptoms.

Associate Professor, Dr. Truong Tuyet Mai, Deputy Director of the National Institute of Nutrition, Ministry of Health, said that overweight and obesity have become a global problem, a global pandemic. Currently, the rate of obesity is increasing vertically.

Experts from the Institute of Nutrition also admit that there are up to 10 factors that gradually cause people to become obese, including: Exercise, nutrition, viruses, hormones, stress, psychology, pollution, technology, food, and social status.

Therefore, the story of controlling overweight and obesity is a challenge, much more difficult than preventing infectious diseases.

The problem here is changing lifestyle and living environment. There needs to be a way to influence policies to solve the living environment for children, and how to create space for children to exercise and play sports.

This is also a contributing factor to why the overweight and obesity situation in children has not improved. In addition, according to Ms. Mai, communication is needed to gradually change living habits in each family.

Overweight and obesity not only affect health, activities, daily life, and psychology, but also must be considered a disease and need to be examined and treated by specialists in digestion and obesity.
