Giant tumor causing difficulty breathing and chest pain

VnExpressVnExpress13/01/2024

Dong Nai A 45-year-old man had chest pain and difficulty breathing. He went to the doctor and was diagnosed with a mediastinal tumor but did not receive treatment. After a year, the tumor weighed more than 2.5 kg and compressed many important organs.

On January 13, Dr. Vo Tuan Anh, Head of the Department of Thoracic and Cardiovascular Surgery, Dong Nai General Hospital, said that the patient was admitted to the hospital because of difficulty breathing, difficulty swallowing, and severe chest pain. CT scan showed a large mass occupying the mediastinum (the cavity in the chest). This mass pushed large blood vessels back, compressed the esophagus and trachea, severely narrowed the main bronchus, and collapsed lung parenchyma.

According to Dr. Tuan Anh, the tumor was large and pressing on important organs, so the dissection had to be very meticulous to avoid damaging these neighboring organs. The surgical team spent more than 180 minutes to completely remove the tumor weighing more than 2.5 kg, while surgeries for small mediastinal tumors took about 60-90 minutes. This is the largest mediastinal tumor ever operated on at the hospital.

One day after surgery, X-rays showed that the patient's left lower lobe of the lung had not expanded, possibly due to phlegm blockage. Doctors examined the patient's lungs with a bronchoscopy and found a lot of phlegm causing bronchial blockage. After clearing the phlegm, the patient's lungs expanded well, the chest pain and shortness of breath were gone, and he was discharged 8 days after surgery.

Doctors recommend that patients who detect mediastinal tumors need early intervention to avoid the tumor growing too large in the chest, compressing organs, affecting quality of life and possibly causing life-threatening complications.

In this case, if surgery is performed earlier, there will be less pressure and invasion of important organs, shortening the surgery time and avoiding complications. After surgery, there is no need for bronchoscopy and sputum suction.
