The hospital wrote "chronic renal failure", the locality only accepted "renal failure"

In recent days, many hospitals in Da Nang have received requests from patients from Quang Nam to confirm their medical condition in order to receive social assistance under Resolution 29.

The irony is that many places that handle policies for people do not accept the medical records that the hospital has diagnosed for the patient but require the correct illness to be recorded according to the resolution.

Having a husband with a serious illness treated at Da Nang Hospital in February 2024 with a medical record of "high blood pressure, kidney failure, old cerebral infarction, bronchitis", Ms. Le Thi Lieu brought documents to Vinh Dien ward, Dien Ban town, Quang Nam province to complete the procedures to enjoy the policy.

Although according to the instructions, policy beneficiaries only need to submit the original or certified copy of the hospital discharge papers and a summary of medical records issued within 12 months, the ward requires them to re-apply for the latest confirmation from the hospital.

Early last December, Ms. Lieu filed a request and the hospital confirmed in writing her medical records: "high blood pressure, kidney failure, old cerebral infarction, bronchitis".

Ms. Lieu brought the papers back but the ward still did not accept them.

The ward government requested that the patient return to Da Nang to request a certificate of illness, stating only "kidney failure" as the disease name in the resolution, and omitting all other related diseases.

"I found it so unreasonable that I told the ward officials that the resolution only required one of the diseases to be eligible for the policy, but my husband had four diseases, so the ward asked the town and said that was the requirement. I had to rush here to get the paper because it was almost expired," said Ms. Lieu.

Similar absurdity also occurred in many districts and towns of Quang Nam province when policy officials asked people to travel many times for unacceptable reasons.

Ms. Ha Thi Sau had heart surgery in 2017 at the Ho Chi Minh City Heart Institute and in 2024 at Da Nang Hospital.

Although the hospital discharge papers were still valid for 1 year, the locality where she lived still required her to go to Da Nang to request a medical record confirmation paper.

Da Nang Hospital has issued a certificate with the content: "Heart valve replacement surgery (mechanical mitral valve replacement surgery, tricuspid valve repair)".

However, the locality where she lives does not accept the above content and requires her to go to Da Nang to get a certificate stating only "heart valve replacement surgery" to match the name of the disease stated in the resolution.

Going to the commune 3 times, then taking the bus to Da Nang 2 more times just because of the surgical method notes and medical records, Mrs. Sau was both exhausted and crying in front of the hospital gate.

"Even a child knows that the parenthesis in the medical record is to clearly state the treatment method, but the local authorities absolutely refused and forced me to go out and ask for the documents to match every word in the resolution. Imagine how much trouble it would have been to travel back and forth if I had surgery far away" - Mrs. Sau indignantly showed the medical records of both heart surgeries.

Resolution to name diseases not in the profession

Like the case of patient Tran Cong Truong (Binh Duong commune, Thang Binh district) who has suffered from chronic kidney failure for many years.

In Mr. Truong's medical record, in addition to the disease "chronic kidney failure", there are many other diseases, but the local government does not accept the disease "chronic kidney failure", but requires him to apply for a certificate confirming the correct disease name in the resolution is "kidney failure".

Twice Mr. Truong had to take the bus to Da Nang in tears because of this absurdity.

These are not isolated cases; almost every day, hospitals in Da Nang have cases of Quang Nam people requesting confirmation of such medical conditions.

As the application deadline in 2024 approaches, many patients have had to urgently contact hospitals in the Central and Southern regions to confirm their medical records.

Many doctors in Da Nang are fed up with the cumbersome and mechanical procedures of localities implementing this policy.

According to Dr. Tran Loc, Deputy Head of the General Planning Department of Da Nang Hospital, on average, more than 20 patients come to the hospital every day to get confirmation papers, of which more than half of the patients have to go to confirm the things clearly stated in the medical records and discharge papers.

Even among the 42 diseases listed in Resolution No. 29, there is a disease that the Ministry of Health's guidelines do not mention, which is the case of "First myocardial infarction".

Doctor Lo said that there are many cases of patients with acute myocardial infarction admitted to Da Nang Hospital. However, localities require people to be confirmed by the hospital according to the disease name in Resolution 29 as "First myocardial infarction".

"The names of the diseases instructed by the Ministry of Health in the treatment regimen are "acute myocardial infarction", "subacute myocardial infarction", "ST-elevation myocardial infarction", "non-ST-elevation myocardial infarction"...

The documents do not mention the concept of "First myocardial infarction" as in Resolution 29.

If it is necessary to adjust the name in Resolution 29 to facilitate patients to enjoy the regime, it will cause difficulties for the hospital because it is not suitable for the profession.

Not to mention, if following this Resolution, how can we confirm how many times a person has had a heart attack to certify it?" - Doctor Loc said.

Speaking with Tuoi Tre Online , Ms. Doan Thi Hoai Nhi, Deputy Director of the Department of Labor, War Invalids and Social Affairs of Quang Nam province, confirmed that there was a cumbersome and rigid implementation of Resolution 29 by some localities.

At the same time, it said it was synthesizing feedback to review and adjust.

Readmission to enjoy the policy

It is worth mentioning that if based on the name in the Resolution, to solve the problem for the people, why is it necessary to request a Summary of the medical record with the Discharge Paper when the Discharge Paper already has the full name of the disease in the diagnosis!?

"Moreover, the request must be granted within 12 months while many people suffer from serious illnesses such as chronic kidney failure, cancer... these diseases are all chronic.

For a long time, people have been having regular check-ups and outpatient treatment at home, so how can they have a discharge certificate (only for inpatients - NV) to make a record? Recently, many patients had to be readmitted to the hospital to meet the conditions to make a record" - a doctor spoke up.
