Hue Central Hospital saves the lightest child with VA-ECMO technique

(Chinhphu.vn) - On March 19, Hue Central Hospital informed that the Hospital's Pediatric Intensive Care and Emergency Department, using the technique of extracorporeal membrane oxygenation (VA-ECMO) and placing a permanent pacemaker, saved the life of a child patient with severe arrhythmia, severe myocardial damage, and risk of death.

The child was hospitalized with symptoms of severe arrhythmia, severe myocardial damage and risk of death.

Patient NNAN (7 years old, residing in Mo Duc district, Quang Ngai province) is the smallest patient (17 kg) and the 6th patient to successfully receive VA-ECMO at the Pediatric Center, Hue Central Hospital.

The child developed symptoms such as fatigue and slow heart rate 3 days before being admitted to the hospital. A few hours after being admitted to the hospital, the child suddenly became very tired, had cold hands and feet, and low blood pressure. Through examination and paraclinical tests, the doctors determined that this was a case of severe arrhythmia, with myocardial damage leading to cardiogenic shock, severe reduction in heart function, and a very high risk of death.

Prof. Dr. Pham Nhu Hiep, Director of Hue Central Hospital, directed the doctors to urgently consult with related multidisciplinary departments to conduct tests and come up with the best emergency plan for the patient. That very night, the doctors and nurses of the pediatric ECMO team, cardiac anesthesia and resuscitation, and cardiothoracic surgery team placed VA-ECMO to support the child's heart.

After 5 days of ECMO, the patient's heart function gradually recovered, but the heart rhythm was still very disturbed (3rd degree atrioventricular block), so the child had to have a pacemaker inserted and be weaned off ECMO.

After more than 2 weeks of treatment, the patient can now walk normally, his heart function has improved, and his hemodynamic and paraclinical indices have returned to normal. The child is expected to be discharged tomorrow (March 20).

After more than 2 weeks of treatment, the patient can walk normally, and his heart function has improved. He is expected to be discharged from the hospital tomorrow (March 20).

Fulminant myocarditis (also known as malignant myocarditis) is a disease with a high mortality rate. The disease has a sudden onset, cardiogenic shock, severe hemodynamic disorders, severe arrhythmias (requiring vasopressors, pacemaker placement, and ECMO support). In Vietnam, the mortality rate of fulminant myocarditis was nearly 100% before ECMO technology was available.

Prof. Dr. Pham Nhu Hiep said: ECMO technique was successfully performed for the first time in Vietnam by Hue Central Hospital in March 2009, mainly for patients with cardiogenic shock, myocarditis, cardiovascular surgery, and progressive acute respiratory failure with ineffective mechanical ventilation.

VA-ECMO is a heart and lung support technique that helps save the lives of many critically ill patients who do not respond to conventional treatments; helping the heart and lungs rest and wait for recovery time. Currently, this technique has been routinely performed at the Pediatric Center of Hue Central Hospital.
