Two consecutive patients hospitalized due to stroke upon waking up

Recently, Xuyen A General Hospital in Ho Chi Minh City said it had received two consecutive cases of stroke upon waking up in the same morning.

The first case was male patient D. who was taken to the emergency room at 3:00 a.m., and the other case was female patient N. who was admitted to the emergency room at 6:00 a.m. the same day.

Through medical history, doctors noted that in both cases, the patients were completely normal before going to bed and when they woke up in the early morning, they found hemiplegia, crooked mouth, and difficulty speaking.

Fortunately, both patients had knowledge of the signs of a stroke so they immediately informed their family members and quickly took them to the emergency room.

After examination, the doctors of the Stroke unit immediately performed a brain MRI and found that both patients had a mismatch between DWI and FLAIR pulses. The doctors consulted and prescribed treatment with thrombolytic drugs to reopen the blocked blood vessels.

Immediately after injecting thrombolytic drugs, the muscle strength of patients D. and N. was significantly improved. A CT angiogram showed that the blood clot had dissolved, but there was still stenosis of a large intracranial artery in both cases.

Therefore, the patients were transferred to the Stroke Unit for further monitoring and rehabilitation treatment. After 48 hours, the muscle strength of both patients had almost completely recovered and they were discharged after 7 days of treatment and regular check-ups to prevent stroke recurrence.

What to do when you detect a stroke while waking up?

From the two cases above, we can conclude that wake-up stroke can still be treated successfully. However, time is the most important factor that determines success. Therefore, the patient must arrive at a hospital with a Stroke unit as soon as possible, before 4:30 from the onset of stroke symptoms, especially in cases of basilar artery occlusion/stenosis.

Unfortunately, there are still many patients who discover suspected stroke symptoms when they wake up in the middle of the night, but they go back to sleep until the next morning before going to the hospital, which delays treatment time and prevents the use of thrombolytic drugs.

According to Dr. CKII Diep Trong Khai - Head of Stroke Unit, recommends: "People need to understand the FAST rule and quickly go to the right hospital with Stroke Unit as soon as they detect suspected stroke symptoms, regardless of time...".

Patients need to have regular medical check-ups to control underlying diseases that can cause stroke. In addition, people need to have a healthy lifestyle, improve their health, limit tobacco and alcohol, to ensure the health of themselves and their family members.

FAST rule to recognize early signs of stroke

F (FACE) crooked mouth : Clearly shown when the patient smiles and shows teeth.

A (ARM) Arm and Leg Weakness : Assess the patient for weakness or paralysis on one side by asking the patient to raise both arms overhead.

S (SPEECH) abnormal language : Ask the patient to repeat a simple phrase. Does the patient understand? Can he or she repeat? Comment on whether the speech is slurred?

T (TIME) when any of the above symptoms appear in a stroke: Call 115 or quickly take the patient to the nearest hospital that treats strokes.
