Saving the life of a 7-year-old patient with acute myocarditis using ECMO technique

(PLVN) - Hue Central Hospital has used the technique of extracorporeal membrane oxygenation (VA-ECMO) and placed a permanent pacemaker to save the life of a child patient with acute myocarditis, severe arrhythmia, and critical cardiogenic shock.

Patient NNAN (7 years old, residing in Mo Duc, Quang Ngai) was admitted to the hospital with symptoms of fatigue and slow heart rate. A few hours after admission, the child suddenly became very tired, had cold hands and feet, and low blood pressure, which progressed very quickly. Through examination and paraclinical tests, doctors determined that this was a case of severe arrhythmia, with myocardial damage leading to cardiogenic shock, severe reduction in heart function, and a very high risk of death.

Under the direction of Prof. Dr. Pham Nhu Hiep, Director of Hue Central Hospital, the doctors urgently held a multidisciplinary consultation with the hospital to conduct tests and come up with the best emergency plan for the patient. That very night, the doctors and nurses of the pediatric ECMO, cardiac anesthesia and resuscitation, and cardiothoracic surgery teams performed VA-ECMO to immediately support the patient's heart.

After 5 days of ECMO, the patient's heart function gradually recovered, but the heart rhythm was still very disturbed (3rd degree atrioventricular block), so the patient was then coordinated with the Cardiovascular Intervention Department to transport the patient to the operating room to place a pacemaker and wean off ECMO.

The patient's heart function is currently improving well and he is expected to be discharged tomorrow (March 20).

After more than 2 weeks of treatment, the patient has gradually improved, can now walk normally, heart function has improved well, hemodynamic and paraclinical indicators have returned to normal and is expected to be discharged tomorrow (March 20). This is the patient with the smallest weight (17kg) and is the 6th patient to successfully receive VA-ECMO at the Pediatric Center, Hue Central Hospital.

Fulminant myocarditis (also known as malignant myocarditis) is a disease with a high mortality rate. The disease has a sudden onset, cardiogenic shock, severe hemodynamic disorders, and severe arrhythmias. In Vietnam, the mortality rate of fulminant myocarditis was nearly 100% before ECMO technology was available.

According to Prof. Dr. Pham Nhu Hiep, the ECMO technique was successfully performed by the hospital for the first time in Vietnam in March 2009, mainly for patients with cardiogenic shock, myocarditis, cardiovascular surgery, and progressive acute respiratory failure with ineffective mechanical ventilation. VA - ECMO technique is a heart and lung support technique that helps save the lives of many critically ill patients who do not respond to conventional treatments; helping the heart and lungs rest and wait for recovery time.
