After examination and testing, the results showed large ureteral stones on both sides causing both kidneys to dilate, the renal parenchyma to thin and lose renal function on both sides, the right kidney was bleeding with blood clots in the kidney, blood tests showed that the patient was very anemic. In this case, additional blood transfusion is needed, and consultation with related specialties such as cardiology, internal medicine and dialysis is needed to stabilize internal medicine. Finally, a program for laparoscopic surgery to remove the right kidney is planned to treat the source of bleeding.

The surgery took place under careful preparation by the team of doctors from the Department of Urology and Anesthesia. Doctor Binh said that the surgery encountered many difficulties due to the large size of the right kidney, causing difficulties in manipulation and obstructing the surgical field. There was a lot of inflammation and adhesion in the space around the kidney, making the dissection relatively difficult and requiring the surgeon to have a lot of experience to avoid damaging the surrounding organs. The team still tried to perform it through endoscopy, although there were many difficulties, but it would help the patient recover as soon as possible after surgery. After more than 180 minutes, the surgery was a great success.

Dr. Vinh said that the patient's initial condition was just bilateral ureteral stones, if diagnosed and treated promptly, the results would be very good. The patient would not have to end up with chronic kidney failure requiring expensive periodic dialysis and would not have to have the kidney removed in a dangerous situation like this case. Although the patient was immediately treated with a catheter to drain both kidneys into the skin, the function of both kidneys could not be improved and was in the final stage of kidney failure. At the same time, the patient's right kidney continued to bleed, causing a lot of blood loss, requiring multiple blood transfusions and eventually the right kidney had to be removed to resolve the cause.

According to Dr. Vinh, urinary stones are a very common disease. According to the US National Library of Medicine, statistics in Southeast Asia show that this region has the highest rate of urinary stones in the world, with a rate of 5 - 19%. In our country alone, about 2 - 12% of the population has urinary stones, of which kidney stones account for 40%. Treatment of urinary stones today is mainly minimally invasive intervention, with quick and gentle recovery. However, if urinary stones are not treated, they will lead to very serious complications, possibly kidney failure, life-threatening infections, and sometimes kidney removal to save lives.
