First time in Vietnam: Removing brain tumor through endoscopic incision on eyebrow

With just a small incision on the eyebrow, doctors at Viet Duc Hospital (Hanoi) successfully performed endoscopic surgery to remove a tumor in the brain.

Going for a check-up due to an accident, Mr. NTH (49 years old, Hanoi) was accidentally diagnosed with a 3cm diameter brain tumor.

Examining the patient, Associate Professor, Dr. Dong Van He - Deputy Director of Viet Duc Hospital - said that the tumor was determined to be benign. However, the tumor was gradually growing and was at risk of compressing the nerves, possibly affecting vision.

After consultation, the doctors decided to perform brain tumor surgery for the patient using endoscopic surgery through the eyebrow arch.

Brain tumor surgery through 3cm incision

According to Dr. He, 20-25 years ago, to intervene in skull base injuries, doctors often had to expand the skull 20-25cm and use a microscope.

Some cases of mid-level lesions can be approached endoscopically through the nose, but this method is not feasible for tumors in difficult-to-reach locations.

"Now with the new method, the doctor only needs to make a small incision in the eyebrow, insert an endoscope to support the light source, observe the lesion and perform surgery to accurately remove the tumor," said Mr. He.

Doctors performed endoscopic surgery through an incision on the eyebrow and successfully removed the tumor. The patient was stable and discharged after 8 days.

Mr. H. shared that after the surgery, he recovered quickly and his health was stable. "When I got home, my relatives did not realize that I had just undergone brain surgery because the incision was very small, almost invisible," Mr. H. said.

Mr. He added that last week, Viet Duc Hospital successfully performed two surgeries to remove meningiomas in the anterior skull base using this technique. Both patients had tumors located right above the roof of the eye socket. This is a new technique that has been successfully performed for the first time in Vietnam.

New technology has more advantages

According to Dr. He, since 2005 Viet Duc Hospital has applied this technique but using a microscope.

Only recently have doctors experimented with incorporating endoscopy into some procedures. This time the entire surgical procedure was performed endoscopically.

This technique has many outstanding advantages such as the endoscope directly accessing the lesion with flexible viewing angles (0 degrees, 30 degrees, 45 degrees), helping the surgeon clearly observe the entire lesion area without much impact on the brain parenchyma.

At the same time, it minimizes the risk of complications and damage to important nerves such as the optic nerve and olfactory nerve. There is no need to expand the brain tissue, reducing the rate of damage to healthy tissue.

In particular, the endoscopic lighting system helps surgeons observe better than traditional microscopes, allowing for accurate assessment of the boundary between tumor and healthy tissue.

Recovery time is faster, patients can be discharged early and return to normal life in a short time.

"This technique can be used to perform priority surgery on brain lesions, aneurysms, second nerve tumors, pituitary craniosynostosis, etc.

With the initial success of this technique, this will be a new direction in minimally invasive neurosurgery, opening up more opportunities for safer and more effective treatment for patients in Vietnam," said Dr. He.

This is also a technique under the state-level research project on minimally invasive neurosurgery, approved by the Ministry of Science and Technology, with the goal of improving treatment methods, minimizing invasiveness and risks for patients.
