Doctors from the Department of Gastrointestinal - Hepatobiliary - Pancreatic Surgery at Bach Mai Hospital have just performed surgery on a case of rectal rupture caused by detoxification using coffee enema.
The patient, Ms. D.TP (38 years old), was admitted to the hospital due to severe abdominal pain in the lower abdomen below the navel accompanied by bloody stools after using a coffee enema to detoxify at a private clinic.
Improper enema causes rectal rupture
The patient had previously used this method twice, one week apart. On the third occasion, during the enema, the patient experienced severe pain in the lower abdomen and bleeding through the anus.
Through medical history, examination and necessary tests, MRI showed an image of retroperitoneal air collection, suspected of rectal rupture. Doctors decided to perform emergency surgery to treat the injury.
During surgery, doctors noted a ruptured lesion in the lower third of the rectum causing a retroperitoneal abscess.
The surgical team sutured the rectal rupture, extensively drained the retroperitoneal space, and created a colostomy for the sigmoid colon. After 14 days, the patient was discharged and was able to eat and live normally. However, Ms. P. will have to undergo another surgery in a few months to close the colostomy.
A surgeon noted that although she was lucky to have survived due to timely surgery, Ms. P, from a normally healthy person, had to endure two serious surgeries, greatly affecting her current and future health, just because she believed in unfounded information and false advertising to sell products and cure diseases.
Inflammation, rupture of colon and rectum
According to Bach Mai Hospital, the online community is spreading a detox method that is said to have many effects such as preventing constipation and curing all diseases. That method is anal coffee enema with the aim of "cleansing" the colon.
The videos instruct viewers to use coffee “specially designed for colon detoxification” and boil it with filtered water. The coffee is put into a bag and hung up high or inserted into a “specialized enema device”, while the other end is inserted into the anus. At this time, the person performing the procedure lies down and relaxes to let the coffee solution flow into the body. Most of these cases are performed at home or in some unlicensed medical facilities.
However, the doctor of the Department of Gastrointestinal Surgery - Hepatobiliary Pancreatic, said that scientific evidence at home and abroad has not confirmed the effectiveness of this detoxification method, and recently the treatment facility has recorded dangerous complications caused by this method. One of them is rectal rupture due to improper enema and can also cause colitis.
In fact, pumping coffee into the anus will reduce the stimulation of the rectum, in the long run will cause loss of rectal reflex, without coffee enema will not defecate.
At the same time, the loss of rectal reflexes also increases the risk of rectal rupture during subsequent enemas, because the patient no longer feels the urge to defecate. Not only that, but administering medication through an unnatural route will push the intestinal microflora into an imbalanced state. Harvard Medical School has warned that bowel cleansing can cause dehydration, electrolyte imbalance, impaired bowel function, and disruption of the intestinal microflora.
