In the first two episodes of The Edge of Evil, the complicated relationship between Jung Se-ok (Park Eun-bin) and Choi Deok-hee (Sol Kyung-gu) is revealed through the past and present. Deok-hee returned to Korea from Boston to work as a professor at a university hospital and met Se-ok, a genius but uncontrollable student. Both began medical school at the age of 17 and shared an obsession with their work. Se-ok, with her outstanding talent in the operating room, developed arrogance and "god syndrome." When Deok-hee recognized her test as "perfect," she yearned to be tutored by him, but her desperation turned into obsession.

A major conflict breaks out when Se-ok loses her chance to go to Boston due to favoritism, causing her to go berserk and get slapped by Deok-hee after causing a ruckus. He forbids her from performing surgery, and a year later, when she performs an emergency surgery in Deok-hee's absence, he slaps her again and vows to revoke her medical license. The climax comes when Se-ok nearly strangles Deok-hee with a tube in front of many witnesses, leading to her kneeling in the rain and screaming in pain as he abandons her. In the present, Deok-hee discovers that Se-ok is performing illegal surgery through a video the police show her, while he hides his brain tumor.

Deok-hee goes to Se-ok, asking her to perform surgery on him so that it won't affect her ability to work, but she refuses and still resents him. He threatens to report her if she doesn't agree, but Se-ok fights back by destroying his medicine when he has an attack. Meanwhile, Se-ok lives a double life: working as a pharmacist by day, performing illegal surgery and even murdering people by night. She coldly eliminates those who stand in her way, with the help of Young-joo (Yoon Chan-young), who manages and covers up her crimes. At the end of episode 2, Deok-hee catches Se-ok burying a victim's body, creating a tense situation that has no end in sight.

The climax comes when Deok-hee, with his talent for psychological manipulation, makes an attractive offer: A research position at a top institution, a chance to regain his lost reputation. He sees this as a "lifeline" for Se-ok, but it is actually a way to bring her back under control, especially when the police are tightening their siege and his health (brain tumor) makes him need her skills more than ever. Se-ok is faced with a choice: Accept the offer to escape the risk of imprisonment or risk going against Deok-hee, even if it leads to self-destruction.
