Just dizzy, nauseous, then discovered vestibular disorder

Ms. NTD (48 years old, living in Binh Thuan) came to the hospital feeling tired and depressed because of headaches, dizziness, nausea... but after more than a year of examination and taking medicine, she still did not get better.


Taking a medical history, Ms. D. said that when the symptoms first appeared, she thought she had cerebral circulatory insufficiency, so she bought brain tonics to take. After 1 month, the symptoms did not improve, so she went to a clinic near her home, was diagnosed with high blood pressure, and took medication as prescribed. The condition did not improve, so she continued to go to a local hospital, where the doctor clinically examined her and diagnosed her with vestibular disorder. After taking the medication for 3 months, the condition did not improve.

Recently, she often had to lie still whenever she felt unbalanced, dizzy, unable to eat or drink, or nauseous. The illness did not go away, she fell into a state of anxiety, panic, insomnia, and had to be treated for anxiety disorder for 4 months.

The symptoms became more and more severe. After being persuaded by her children, Ms. D. agreed to go to Tam Anh General Hospital in Ho Chi Minh City for examination.

On December 15, Master - Specialist Doctor Tran Thi Thuy Hang, Head of the Department of Otolaryngology, Otolaryngology Center, Tam Anh General Hospital, Ho Chi Minh City, said that after clinical examination and medical history, the patient's vestibular function was measured using a system that combines dynamic eye imaging technology using artificial intelligence. With 18 pre-programmed measurement methods, Ms. D. was tested for vestibular eye movement, tested for head impulses, analyzed the vestibular-ocular reflex function of the 6 semicircular canals, and tested for positioning to analyze nystagmus in different positions.

The results showed that the patient had benign paroxysmal positional vertigo, with ectopic otoliths in the posterior semicircular canal - a form of peripheral vestibular disorder.

"Ectopic ear canal stones can disappear on their own within a few days or weeks, but can progress to a chronic condition like in the case of patient D," Dr. Hang shared.

The patient was instructed by the doctor on the otolith repositioning exercises. She went to the hospital to practice 4 times, and also practiced at home following the doctor's instructional video. She no longer had dizziness, spinning, or headaches after 3 weeks of treatment.

Ms. D. had her eye movements tested with AI-based dynamic eye chart technology.

Do not take medication without an accurate diagnosis.

According to Dr. Hang, many patients with vestibular disorders are tired and confused because each examination results in a different diagnosis. The center also regularly receives many patients with vestibular disorders who come for examination after a long period of treatment without success, mainly because they have not treated the cause properly or self-medicated; in some cases, because of anxiety and panic, they switch to additional treatment for anxiety disorders.

Peripheral vestibular disorders can be completely cured, limiting the risk of recurrence, avoiding long-term complications if the patient is treated properly and actively. If the patient is suspected of having brain damage, the neurologist will prescribe some additional paraclinical tests, measuring vestibular function, measuring ear function, CT scan of the ear, MRI of the brain, etc.

“Arbitrary use of drugs without proper diagnosis is not only ineffective but also carries many risks. To effectively treat vestibular disorders, patients should see the right specialist to determine the exact cause; choose a reputable facility with modern equipment and multidisciplinary coordination; comply with the doctor's treatment; and return for follow-up visits as scheduled. At the same time, patients should also have a healthy lifestyle and diet; control stress; maintain adequate sleep; and limit sudden changes in head position," Dr. Hang advised.
