MEDLATEC General Hospital has just admitted a male patient, D.VT (63 years old, Hanoi), who has a history of chronic hepatitis B for more than 10 years but subjectively did not seek treatment.

Hepatitis B progresses to liver cancer

When he experienced left chest pain and abdominal distension, the man went to the doctor and was diagnosed with liver cancer. A week before being hospitalized, Mr. D.VT had symptoms of left chest pain, the pain did not spread, did not increase with exertion, and did not have difficulty breathing. In addition, the patient had abdominal pain, bloating, and indigestion.

Medical history showed that the patient had a history of chronic hepatitis B for more than 10 years but had not had regular check-ups or specific treatment, and regularly drank alcohol, about 120 - 200ml per day. Notably, Mr. T's family had a biological mother who had liver cancer and passed away 10 years ago, and his eldest brother also passed away 8 years ago.

The doctor ordered the patient to do the necessary tests and imaging. The HBeAg test result was positive, HBV DNA viral load was high. Abdominal ultrasound detected a hyperechoic mass in the right liver, measuring 60x53mm, liver tissue elastography concluded that the liver parenchyma was coarse.

Continuing to perform a computed tomography scan, the image of a hypervascular mass in the right segment VIII of the liver (LIRADS 4) and some atypical enhancement nodules in the right liver/heterogeneous liver parenchyma. To confirm the diagnosis, the doctors performed a liver tumor biopsy and sent the pathology sample. The pathology results concluded that it was moderately differentiated hepatocellular carcinoma.

Based on the results of the paraclinical tests performed, the doctor diagnosed the patient with hepatocellular carcinoma (HCC).

Hepatitis B is a global health threat and is a common liver infection in the world. The disease is caused by the hepatitis B virus (HBV), common symptoms include fatigue, loss of appetite, nausea, yellow eyes, yellow skin, dark urine, abdominal pain, muscle and joint pain, unusual weight loss...

According to statistics from the World Health Organization (WHO), there are currently 257 million people with hepatitis B in the world (equivalent to 8.1% of the population).

Estimated data in Vietnam in 2020 showed that there were 8.9 million cases of chronic hepatitis B, 58,650 patients with cirrhosis, 25,000 patients with liver cancer and up to 40,000 people died each year from this disease.

Dr. Ngo Chi Cuong, Deputy Director, Head of Internal Medicine Department, MEDLATEC General Hospital, Head of Infectious Diseases & Tropical Medicine Department, Medlatec Healthcare System said that if not treated promptly and properly, hepatitis B can progress to a chronic state, leading to many serious health problems such as cirrhosis, liver failure, liver cancer. In particular, patients with hepatitis B have a risk of liver cancer 100 times higher than normal, with a very high mortality rate.

Faced with alarming figures about the current state of hepatitis B, Dr. Cuong recommends that people should be proactive in having regular health check-ups and performing screening tests to detect hepatitis virus.

Depending on the stage of the disease, doctors will have appropriate treatment plans. The time of disease detection determines the effectiveness of treatment, limits dangerous complications and saves costs for patients.

Patriotic Hygiene Day to Improve People's Health 2024

Hanoi Department of Health has just issued plan No. 3659/KH-SYT on responding to Patriotic Hygiene Day to improve people's health in 2024.

The purpose of the plan is to raise awareness and promote the responsibility of all levels of government, unions, social organizations, each agency, unit, family and the whole society about the patriotic hygiene movement to improve people's health.

The organization of response activities must be practical and effective, according to the instructions of the Department of Health. The plan clearly states 4 contents of activities responding to the Patriotic Hygiene Day to improve people's health in 2024.

Firstly, propaganda to raise awareness and behavior to properly solve health-related hygiene issues such as hygiene to prevent epidemics, food safety; practice clean eating, clean drinking, and clean living;

At the same time, collect and do not litter, maintain hygiene and environmental landscape; build and use hygienic toilets; build hygienic barns and move livestock and poultry barns away from homes; build green - clean - beautiful medical facilities, ensure hygiene in schools, production and business establishments, agencies, service areas, tourist areas, and public places.

Second, based on local conditions, the unit organizes environmental sanitation campaigns, cleans sanitation facilities in public places, tourist areas, hospitals, medical facilities, schools, workplaces, etc.; organizes general cleaning of village roads and alleys, builds civilized lifestyles, cultural families associated with the movement "All people build new rural areas".

Third, organize and launch competitions in response to the patriotic hygiene movement to improve people's health in residential areas, schools, agencies and offices. Direct local medical facilities to participate in the "Green - Clean - Beautiful Medical Facility" competition according to the instructions of the Ministry of Health.

Fourth, strengthen inspection and supervision of hygiene and disease prevention, personal hygiene, food safety, quality of clean water for daily use; occupational hygiene in production and business establishments, environmental hygiene in medical facilities and schools; disease prevention and control in hotspots, areas at risk of disease outbreaks, flooded areas, storms, etc. to ensure effectiveness and compliance with current regulations.

Assign the City Center for Disease Control to be the focal point to guide units in the industry to deploy and organize activities in response to the Patriotic Hygiene Day to improve people's health in 2024 according to the instructions of the Ministry of Health.

Coordinate with media and mass media agencies to develop and provide content and propaganda articles to raise public awareness of Patriotic Hygiene Day and the Patriotic Hygiene Movement, and improve people's health on the media.

Coordinate with units to organize inspections of disease prevention, environmental sanitation, and monitor the quality of clean water used for domestic purposes in the area according to decentralization.

Receive and summarize the response results of units in the health sector and report to the Department of Health and the Department of Medical Environment Management (Ministry of Health) according to regulations.

Hanoi Food Safety and Hygiene Department guides units in the industry to deploy and organize activities to ensure food safety.

Strengthen the implementation of communication and training activities on food safety and food poisoning prevention in schools, industrial parks, tourist areas... Coordinate with units to organize inspections of food safety work in the area according to decentralization.

The Department of Health also requested medical examination and treatment units in the industry to develop plans and organize activities to respond to the Patriotic Hygiene Movement to improve people's health in 2024.

Organize and launch competitions in response to the Patriotic Hygiene Movement to improve people's health, participate in the "Green - Clean - Beautiful Medical Facility" contest according to the instructions of the Ministry of Health.

Hospital Management of Chronic Obstructive Pulmonary Disease and Asthma

In Vietnam, about 4.1% of the population has bronchial asthma, however only 29.1% of asthma patients receive maintenance treatment and the rate of good asthma control is less than 40%, the average number of asthma attacks per year is 2.14%.

With chronic obstructive pulmonary disease, the rate of Vietnamese population aged 40 and over suffering from the disease is 4.2%, this is also the 4th leading cause of death in our country.

To improve the quality of asthma and chronic obstructive pulmonary disease management in Hanoi, Dr. Nguyen Dinh Hung, Deputy Director in charge of the Hanoi Department of Health, proposed that health facilities should increase screening activities for asthma and chronic obstructive pulmonary disease in the community; establish more outpatient asthma and chronic obstructive pulmonary disease management units.

At the same time, hospitals need to develop a diagnostic and treatment protocol for asthma and chronic obstructive pulmonary disease to help standardize the examination and treatment process for asthma and chronic obstructive pulmonary disease, from screening, diagnosis to maintenance treatment and acute exacerbation management.

On November 1, 2023, the Ministry of Health issued Decision No. 4032/QD-BYT on the pilot issuance of "Criteria for assessing the quality of diagnosis, treatment and management of Asthma and Chronic Obstructive Pulmonary Disease" with the aim of establishing a management system to improve the quality of Asthma and COPD management, ensuring that patients can access good health care, control and reduce the burden of disease.

Recently, the Department of Health directed the Hanoi Lung Hospital to conduct a survey with hospitals in the area to evaluate this criterion.
