9-year-old girl in deep coma due to rare diabetes

On September 19, information from Ha Tinh General Hospital said that doctors at this hospital had just saved the life of a 9-year-old patient who was in a deep coma and in critical condition due to diabetic ketoacidosis.

Accordingly, patient NTKN (9 years old, residing in Huong Khe Town, Huong Khe District, Ha Tinh) was admitted to the Pediatrics Department of Ha Tinh Provincial General Hospital with symptoms of coma, difficulty breathing, and rapid shallow breathing.

The patient's family said that 3 weeks ago, the child lost weight very quickly, from 37 kg to 32 kg without any clear reason. Recently, the child showed symptoms of vomiting and fatigue, so the family took the child to the district hospital and then transferred him to the provincial general hospital.

After admission, the patient's condition worsened, so he was given oxygen through a mask, intubated, ventilated, placed on invasive arterial blood pressure monitor to continuously measure blood pressure, placed on a central venous catheter...

Through emergency blood tests, doctors discovered that the TM blood sugar index was 59 mmol/l (normal is 3.4-6.2 mmol/l), capillary blood sugar could not be measured (HI), blood gas pH was 6.8 and the patient was diagnosed with diabetic ketoacidosis, and was ordered to monitor for sepsis.

After 1 day of mechanical ventilation, blood sugar and insulin adjustment, the patient's blood sugar index returned to 17 mmol/l and the severe ketoacidosis index returned from pH 6.8 to normal (pH 7.36). The patient was also conscious, extubated and continued to be consulted by the medical team with the National Children's Hospital to continue treatment according to the protocol. After consultation, the doctors agreed to switch to insulin injection dose of 4 injections/day.

According to Dr. Duong Van Giap, Deputy Head of the Pediatrics Department, Ha Tinh General Hospital, in children, diabetes is mainly type 1, also known as insulin-dependent diabetes, because the pancreas is no longer able to produce enough insulin. Although diabetes in children is rare, complications of ketoacidosis coma can progress severely, leading to death if not detected early and treated promptly.

"If parents find that their children eat a lot, drink a lot, urinate a lot, lose weight, etc., they may have diabetes, so they should take their children to a medical facility with a specialist for examination. To prevent diabetes in young children, parents also need to have a healthy, scientific diet, rich in fiber and increase their children's physical activity," Dr. Giap recommended.
