Intervention for a girl with a mass that swells when awake and crying but deflates when sleeping

The baby girl is only 10 months old and was diagnosed with bilateral Nuck's canal hernia. Doctors at Children's Hospital 2 (HCMC) have just performed early intervention for the baby.

On March 12, Children's Hospital 2 announced that its doctors had just applied advanced early intervention techniques for a 10-month-old girl named TML, living in Ho Chi Minh City, who had bilateral Nuck's canal hernia.

Baby L. was taken to the nephrology department for examination because of a swelling in the groin and labia majora on both sides, the right side being larger than the left.

This bulge tends to get bigger when the baby is awake and crying, but goes down when the baby is asleep. Whether it gets bigger or goes down, the child is almost painless.

The patient was diagnosed with bilateral Nuck's canal hernia, of which the right side was a sliding hernia with the herniated organs being the fallopian tube and right ovary. The child was scheduled for early surgery.

During surgery, the herniated organs are returned to the abdomen and the two Nuck tubes are cut and sutured closed.

The patient was discharged the same day and the surgical wound healed well 7 days postoperatively.

Doctor Phan Le Minh Tien, Department of Nephrology (Children's Hospital 2), said that Nuck's canal hernia is a congenital malformation caused by the abnormal existence of the Nuck's canal - an embryonic structure similar to the peritoneal canal in boys.

Unlike a common inguinal hernia, a Nuck canal hernia can be more serious because it often involves vital organs such as the ovaries and fallopian tubes. This increases the risk of complications such as ovarian strangulation or torsion, making early detection and treatment very important.

Signs of Nuck's hernia in girls

Dr. Pham Ngoc Thach, deputy director of Children's Hospital 2, said that Nuck's canal hernia can manifest as a bulge in the groin area, which can appear when the baby moves or cries and sometimes disappears on its own when resting.

If the hernia contains an ovary or fallopian tube, there is a risk of twisting and ischemia, leading to groin pain and possibly permanent damage to the reproductive organs. Therefore, parents need to closely monitor for unusual signs and take their children to the doctor immediately if there is any suspicion.

Previously, Nuck canal hernia surgery mainly used absorbable sutures to close the hernia site. However, with the development of medical technology, biological glue has been applied in surgery as an alternative method with many outstanding advantages such as helping wounds heal beautifully, reducing the need for wound care, limiting the risk of infection and not needing to remove stitches.

Nuck's canal hernia in girls can have serious consequences, especially when the ovaries and fallopian tubes are involved. Early detection and surgery play an important role in preventing complications and protecting the child's reproductive function.
