
On April 4, MSc. Dr. Luu Phuong Lan, Department of Plastic Surgery - Microsurgery, 108 Central Military Hospital, said the patient suffered serious injuries with a large exposed skull area and a severed scalp that was no longer intact.
According to Dr. Lan, microsurgical scalp re-grafting is the optimal solution in terms of both function and aesthetics. However, this method is not always feasible, especially when the severed skin cannot be brought to the hospital.
In this case, the doctors planned to reconstruct by using a skin flap from the back, the largest size up to 10x41 cm, to cover the exposed occipital bone and ensure load bearing when the patient lies down. This skin flap is rotated 180 degrees, extending to cover the damaged area on the right occipital skull and the left parietal. Due to the large area of the scalp loss, a large amount of thin skin from both thighs was also taken to graft to supplement the remaining areas.
The surgery was difficult, especially when fixing the grafted skin and maintaining the patient's proper lying position to avoid putting pressure and tension on the skin flap. Fortunately, the graft was successful, the patient was stable, able to sit up and walk.

When giving first aid to a victim, it is necessary to properly preserve the severed parts, gently wash them with clean water and then wrap them in gauze or damp cloth. Store the severed parts in a bucket or ice pack with an ideal temperature of 4-5 degrees Celsius, do not let them come into direct contact with ice, and bring them to the hospital for transplantation to the patient.
