Technique to help women with low egg count have children

With the technique of egg freezing, embryo creation, and embryo culture using artificial intelligence, women with severe ovarian failure still have the chance to get pregnant and give birth to healthy children.

That was the sharing of MSc. Dr. Giang Huynh Nhu - Director of the Center for Reproductive Support, Tam Anh General Hospital, Ho Chi Minh City (IVFTA-HCMC), in the consultation program "Female infertility due to ovarian failure, reduced ovarian reserve (low AMH) and egg storage for motherhood", on the evening of May 25. MSc. Dr. Giang Huynh Nhu said that the best age for women to get pregnant and give birth is 20-30 years old, when the number of eggs in the ovaries (AMH index) is abundant and of the best quality. As women get older, the ovaries will gradually age, decreasing in quantity and quality until menopause, ovarian exhaustion. However, there are cases where women have premature ovarian failure, at a very young age, due to diseases related to the ovaries, fallopian tubes or congenital.

According to 2022 statistics at IVFTA-HCMC, 47% of patients coming for examination have AMH below 1.2, meaning they have low ovarian reserve. This is a challenge because the egg cell is the cell that determines the success and embryo creation for the entire treatment cycle.

However, thanks to egg collection and egg storage techniques combined with the ISO 5 Lab embryo culture system integrated with artificial intelligence software, these women still have the chance to become mothers.

"Egg cells are difficult to store because after storage and thawing, they can survive in form but cannot be used in function anymore. At IVFTA-HCMC, with the super clean ISO 5 Lab system, integrated with artificial intelligence software, we are confident in mastering this technique," said MSc. Dr. Giang Huynh Nhu.

Normally, the pregnancy rate in IVF is often divided by age group such as 29 years old, 29-35 years old, 35-40 years old and after 40. At IVFTA, statistics at the end of 2022, the groups under 40 years old all have nearly the same pregnancy rate, about 60-65%, thanks to good egg freezing and embryo making techniques with both frozen and fresh eggs. There are patients with AMH of only 0.1 who still get pregnant successfully and give birth without having to ask for eggs from others.

Dr. Pham Thi My Tu said that not long ago, she received a fairly young patient whose AMH was only 0.1. The patient had done IVF 3 times at other hospitals but failed. After persevering for 5 months with 4 cycles of ovarian stimulation and egg retrieval, the patient had 4 good quality embryos, increasing the chance of pregnancy. The patient can also continue IVF to have a second and third child smoothly many years later, even when the ovaries are exhausted.

In response to the concerns of many young readers about how to recognize ovarian failure in order to examine and treat it promptly, Dr. Tu recommends that the menstrual cycle of a normal healthy woman is 28-32 days. Women of reproductive age with menstrual disorders, sparse or decreasing menstrual cycles... should go to the hospital for examination, even if they are single or married.

If ovarian failure is diagnosed, egg freezing is considered "reproductive insurance". When storing eggs, the egg's biological clock will stop at that time. Egg quality is maintained, helping women preserve their ability to become mothers in the future, proactively planning pregnancy and giving birth.

Dr. Ngo Dinh Trieu Vy said that at IVFTA, there are also many female clients who want to become mothers who have come to store eggs before using hormone therapy or gender reassignment surgery. In cases where hormones have not been suppressed or have stopped being suppressed, and have ovaries and uteruses, eggs can be stored. Then, sperm from a donor or from a sperm bank can be used for IVF and pregnancy and normal birth.

Unmarried women, focusing on their careers and personal development, planning to have children in the future but have not yet determined a specific time; or women who want to be single mothers should also consider the method of egg freezing to preserve fertility as soon as possible. In younger women, the number of eggs collected is higher, the quality of eggs is better, increasing the chance of success when creating embryos and transferring embryos.
