17-year-old male student has 'rich people's disease', doctor warns of cause

For 2 years, the 17-year-old male student had a dull pain in his toes, but did not seek medical attention. When the pain started to get worse and he became hot and swollen, he went to the hospital and was diagnosed with chronic gout.

17 years old and already has 'rich man's disease'

Student LMH (male, 17 years old, in Hanoi) came to MEDLATEC General Hospital for examination due to pain in the first metatarsal joint of the left foot and was diagnosed with acute gout. After 1 week of treatment, the pain was gone.

H.'s parents shared that for the past 2 years, he has had many episodes of dull pain in the first toe of his left foot, and the pain when walking or moving around has gone away after 3-5 days.

This time (2 days after being admitted to the hospital), H. had severe pain in the first metatarsal joint of his left foot, swelling and heat, continuous pain, increased with movement, sometimes at night. H. also had no history of trauma, no pain in the spine or other joints, and had not been treated with medication, so he went to see a doctor.

Regarding personal and family history, parents said that H. had tetralogy of Fallot - the most common congenital heart disease - and had surgery more than 15 years ago, so he had regular cardiology check-ups and was allergic to antibiotics (Ceftriaxone). Family history includes a maternal grandfather with gout.

According to Dr. Trinh Thi Nga, a musculoskeletal specialist, this disease is often called "rich people's disease" so there are many subjective cases, the case of student H. is an example.

According to Russian doctors, there are many causes of gout such as a diet high in protein and seafood, age and gender, drinking a lot of beer and alcohol for a long time, obesity, and having a family member with gout.

In the case of child H., who has a normal diet, is short (due to congenital heart disease), and is young, when unusual signs appeared, the family subjectively thought it was caused by changes in the weather.

However, because H. has congenital heart disease with cyanosis, he is at higher risk of gout and increased uric acid, and his grandfather has gout as a genetic factor. Therefore, H. needs to be examined and checked periodically every 3-6 months to best control gout.

What are the signs that you have gout?

Gout, also known as gout, is a disease caused by a disorder of purine metabolism in the kidneys that makes the kidneys unable to filter uric acid from the blood.

Uric acid is usually harmless and is formed in the body and then excreted in urine and stool. In people with gout, uric acid builds up in the blood over time.

When this concentration is too high, small crystals of uric acid are formed. These crystals collect in the joints and cause inflammation, swelling and pain for the patient.

"Although it is a benign disease, if not detected and treated promptly, the disease can become serious, such as tophi, joint damage, kidney stones...

In particular, young people with gout can lead to cardiovascular diseases such as myocardial infarction, stroke, diabetes, chronic kidney disease...", said Dr. Nga.

According to Russian doctors, when signs of pain appear such as sudden, severe joint pain, swelling, redness, warmth around the joint..., most of these signs usually last for several hours in 1-2 days, to detect the disease promptly, people need to see a doctor immediately.

From the examination results, the doctor advises on a healthy diet and lifestyle to avoid serious disease progression, consequences and effects on quality of life.

To prevent gout, people should build a healthy lifestyle to reduce the risk of uric acid accumulation and acute gout attacks by drinking plenty of water, limiting alcohol, maintaining a reasonable weight, not eating too many foods rich in nutrients and having regular health check-ups.
