Obesity-related sleep apnea

On April 3, the Central Endocrinology Hospital informed that doctors there had just received a male patient TTĐ (28 years old, Hanoi ) weighing 175kg in a state of difficulty breathing, heart failure and swelling in both legs leading to the inability to move. Mr. Đ was quickly supported with a ventilator and received special care in the Intensive Care Department.

It is known that 10 years ago, D was diagnosed with obesity and chronic gout. In the 2 weeks before being hospitalized, D gained more than 10kg of uncontrolled weight due to consuming a lot of sweet drinks (milk tea, soft drinks, etc.).

MSc. Dr. Nguyen Dang Quan, Deputy Head of the Department of Intensive Care, said: "Through clinical examination and test results, patient D was diagnosed with sleep apnea syndrome. This syndrome is common in patients with a history of cardiovascular diseases and metabolic disorders such as obesity, gout, type 2 diabetes, high blood pressure, heart failure, stroke...".

Common symptoms of obstructive sleep apnea include:

+ Snoring: This is the most common sign. Patients have nocturnal apnea, wheezing, gasping and terminal apnea. Snoring is loudest when lying on the back and decreases when lying on the side.

+ Tired all day: People with sleep apnea are often tired, have difficulty concentrating at work, have memory loss, mood swings, and are easily irritated.

+ Daytime sleepiness: Patients may fall asleep while working, even while driving.

+ Headache when waking up: Caused by reduced brain oxygen concentration at night.

In overweight and obese people, fat is distributed around the upper respiratory tract, leading to narrowing of the airway. This contributes to increased airway compression during sleep. In addition, fat concentrated in the chest and abdomen also reduces lung capacity and increases oxygen demand. Therefore, obstructive sleep apnea is the most common disease, especially in overweight and obese patients. If not treated promptly, it will cause many dangerous complications such as myocardial infarction, stroke, high blood pressure, etc.

According to Dr. Quan, some symptoms of obstructive sleep apnea can be confused with other diseases, so they are not recognized early, are unclear and are often overlooked. Therefore, diabetic patients in particular and patients at risk in general need to go for a medical examination to detect this syndrome early to limit unnecessary complications.

Obesity is the culprit of 200 other diseases

According to Dr. Lam Van Hoang, Director of the Weight Loss Center at Tam Anh Hospital, Vietnam is currently ranked 197th in the world in terms of overweight and obesity (according to WHO). However, the rate of people with the disease is increasing rapidly, from 3% to 15%, making it one of the countries with the fastest increasing obesity rate in the world.

Dr. Hoang added that if calculated by waist-hip ratio, currently 1 in 2 adults is obese, while weight control is still not recognized and properly intervened.

Unlike other common diseases, obesity is the culprit of more than 200 different diseases, such as: cardiovascular disease, stroke, diabetes, osteoarthritis, fatty liver and many cancers, especially gastrointestinal cancer...

However, because the progression of these diseases is often silent and does not manifest immediately, many people are negligent, subjective, do not go for check-ups, and do not control their weight properly.

Obesity is a complex disease with many causes and factors, including biological (pathological), psychosocial, lifestyle, and nutritional factors. Therefore, to treat overweight and obesity, it is not only about losing weight but also reducing the risk of complications and improving health. Good treatment of overweight and obesity, losing 5% -15% of weight in about 6 months has been proven to bring health benefits, prevent complications due to overweight and obesity, and at the same time repel co-morbidities.
