Investor speaks out about initial cause of box culvert collapse accident
Accordingly, at 9:30 a.m. on November 8, at the construction site of a drainage box culvert for the residential road (Km464+720, right of way) belonging to Package XL-01, Dien Chau - Bai Vot expressway project, in Hung Dao commune, Hung Nguyen district, Nghe An province, a work accident occurred, resulting in the death of two construction workers.
The incident occurred during the process of dismantling the culvert wall formwork of the contractor Dai Hiep Limited Company. The contractor/project enterprise of package XL-01 - where the accident occurred is Phuc Thanh Hung Investment Joint Stock Company.
According to the contractor representative, the initial cause of the incident was assessed by all parties as possibly due to the recent prolonged rainy weather, which caused the ground around the foot of the main wall to become saturated. The exact cause is currently being clarified by relevant authorities.
Immediately after this incident occurred, the Ministry of Transport requested relevant units to suspend the contractor's construction and clarify the cause and responsibility in this accident.
It is worth mentioning that just 6 months ago, also at the construction site of the Dien Chau - Bai Vot expressway project, also invested by Phuc Thanh Hung Investment Joint Stock Company, there was a bridge girder collapse incident due to the contractor's negligence.
The scene of the bridge girder collapse was due to negligence by the contractor at the Dien Chau - Bai Vot expressway component project in mid-May.
Specifically, on May 19, during the construction of Nghi My bridge girder (Km 448+141) under package XL-02, when the contractor was installing the last girder of the span of pier T3 - pier T4 of Nghi My bridge, a collision occurred, causing two girders to collapse and four installed girders to tilt.
The parties involved determined that the cause was due to the crane operator's negligence and uncoordinated operations, leading to the collision. The contractor used its own funds to construct new replacement beams to ensure the quality and progress of the project.
Immediately afterwards, the Ministry of Transport inspected and requested the project enterprise to review and immediately issue inspection and quality control procedures if any were missing; requested contractors to strictly implement them, absolutely not allowing similar situations to happen...
The Ministry also reported to the Prime Minister about this incident, affirming that construction quality control is always the top priority and requested that the subjects participating in the construction investment process must uphold their responsibilities and strictly implement regulations on construction investment management as well as instructions and directives of the Minister of Transport.
The fact that two incidents occurred only 6 months apart at the same project with the same contractor/project enterprise has created concerns about the quality of the North-South expressway, from the section passing through Nghe An province .
