Candidates are students in grade 12, fill out online registration information.

The Ministry of Education and Training notes that before filling out the exam registration form, candidates must carefully read the sections and the INSTRUCTIONS FOR FILLING OUT THE FORM. If there are any unclear points, candidates must ask the exam registration reception staff for full instructions. Candidates are fully responsible for the information declared in the exam registration form.

Candidates who are students in grade 12 will fill out the online registration information using the account provided on the Exam Management Software System. After completing the review and confirmation within the prescribed registration deadline, the High School where the candidate is studying will print the Registration Form, Form No. 1, Form No. 2 and sign and stamp a seal with a border on the photo of the Registration Form to confirm the candidate's identity.

How to fill out the form for people who have a high school diploma to register for university and college entrance exams

People who do not have a high school diploma or have a secondary school diploma taking the exam to graduate from high school or using the high school graduation exam results as a basis for registering for university or college admission must fill in all items on the software.

People who have a high school diploma or a secondary school diploma and register to take the exam to use the high school graduation exam results as a basis for applying for university or college admission only need to fill in items 1 to 15 and items 24, 25, 26, 27 on the software.

The place receiving the registration keeps the envelope containing the registration form, Form No. 1, a copy (photocopy) of the CCCD/ID card and 2 photos, and returns Form No. 2 to the candidate after signing and stamping the confirmation.

Candidates keep this Form No. 2 to receive the Exam Notice and Certificate of Exam Results. In case of any errors in the exam registration information or loss of the Exam Notice, candidates must bring this Registration Form directly to the registered Exam Location at the meeting to disseminate the regulations to request correction of errors and complete the exam procedures.

How to fill in 27 items on the 2024 high school graduation exam registration form

Section DEPARTMENT OF EDUCATION AND TRAINING.........DEPT CODE : Candidates registering at the exam registration unit under which Department of Education and Training, write the name of the Department of Education and Training in the blank space, then fill in the 2 digits representing the Department of Education and Training code in the next 2 blank boxes, the Department of Education and Training code is specified by the Ministry of Education and Training.

Number of tickets : The place where the exam registration is received must be filled in. Candidates must not fill in this section.

Section 1, 2 : Fill in according to the instructions on the High School Graduation Exam Registration Form (hereinafter referred to as the Registration Form).

Section 3 : a) The candidate's place of birth only needs to clearly state the name of the province or centrally-run city (province/city). If born abroad, the candidate only needs to clearly state the name of the country (in Vietnamese). b) The ethnicity is recorded correctly according to the birth certificate. c) For foreign nationality , mark (X) in the box next to it.

Section 4 : Citizen Identification Number, National Identity Card Number, Personal Identification Number and Passport Number are written together as Citizen Identification Number/National Identity Card (CCCD/CMND) in this section. For the new model CCCD/CMND, write all 12 digits in the corresponding boxes; for the old model ID card, write 9 digits in the last 9 boxes on the right, leave the first three boxes blank.

Section 5 : The province/city code, district/county code and commune/ward code for communes/wards in Region 1 will be determined by the Ministry of Education and Training. Candidates need to look up at the place of registration to correctly enter the province/city code, district/county code, commune/ward code where the candidate has a permanent residence in the corresponding boxes on the right. Candidates who do not have a permanent residence in a commune in Region 1 should leave the commune code box blank. After filling in all administrative unit codes in the boxes, candidates clearly write the name of the commune/ward, district/county, province/city in the blank line.

For candidates who are prioritized in subjects or areas related to their permanent residence, it is required to confirm that they have had a permanent residence of over 18 months in area 1 or over 18 months in a particularly disadvantaged commune or a commune with a particularly disadvantaged village during their high school years by checking the corresponding box.

Section 6 : Write the school name and address to the district/county, province/city of the school on the dotted line. Write the province code where the school is located in the first 2 boxes, write the school code in the next 3 boxes (school code is written according to the regulations of the Department of Education and Training, if the school code has 1 digit, write the number 0 in the first 2 boxes, if the school code has 2 digits, write the number 0 in the first box).

For candidates who are police officers or soldiers sent to take the exam for university and college admission, write the province/city code corresponding to the province where they are stationed and the high school code is 900.

For candidates who have studied abroad, the years of study abroad must be recorded as the province/city code corresponding to the province/city of their permanent residence in Vietnam and the high school code is 800. Grade name section: clearly state the name of the 12th grade where the student is studying (for example 12A1, 12A2,...), for independent candidates, write "TDO".

Section 7 : Clearly state your phone number and email. For candidates who request to adjust their online admission registration, they need to register their mobile phone number to receive a one-time password (OTP) via text message to ensure security when registering online.

Section 8 : Candidates must clearly state the contact information: full name; phone number; hamlet address (house number), village (street, alley), commune/ward, district/county, province/city. This address is also the address to receive the Admission Notice if the candidate is admitted.

Section 9 : Candidates who wish to use the exam results to consider admission to university or college in Early Childhood Education, please mark (X) in the box next to it.

Section 10 : Candidates must mark (X) in one of the two boxes to clearly indicate whether they are studying under the high school program or the high school continuing education program.

Section 11 : For independent candidates, mark (X) in one of the two boxes to clearly distinguish whether the independent candidate has not graduated from high school or has graduated from high school (at the time of the exam).

Section 12 : Candidates registering to take the exam at which examination council must write the name of the examination council and the examination council code prescribed by the Ministry of Education and Training in the corresponding position.

Section 13 : Students studying in grade 12 of high school must submit their registration at that school. Other subjects must submit their registration at locations specified by the Department of Education and Training. The registration unit code is recorded according to the instructions of the place receiving the registration.

Section 14 : For candidates who are currently grade 12 students (not graduated from high school), they must register for the exam at point a. Candidates are not allowed to choose the component subjects at point b.

For independent candidates, depending on the purpose of the exam, depending on the choice of university and college entrance exam subjects, they can choose the whole exam (at point a) or only choose some component subjects (at point b) as appropriate. Candidates can only register for one combined exam (Natural Sciences or Social Sciences). Independent candidates can only register for component subjects in the same combined exam.

In case the independent candidate has not graduated from high school and has exams/subjects (for high school graduation recognition) from the previous year that are eligible for retention, if he/she wants to retain any exam/subject, he/she must record the score of that exam/subject in Section 16. However, the candidate can still choose to take the component exam/subject (that he/she requested to retain) to get the results for university and college admission.

Candidates studying under the GDTX program can choose the Foreign Language exam if they wish to use the Foreign Language subject in the subject group for university and college admission.

How to choose a component test/subject: Candidates who register for a component test/subject should mark (X) in the corresponding component test/subject box. For the Foreign Language test, candidates should fill in the code corresponding to the specific language as follows: N1 - English; N2 - Russian; N3 - French; N4 - Chinese; N5 - German; N6 - Japanese; N7 - Korean. Candidates can only choose component tests in a combined test.

Section 15 : For candidates wishing to be exempted from the Foreign Language exam, it is necessary to clearly state the type of certificate that qualifies for exemption or clearly state that they are a member of the national team participating in the International Olympic competition in Foreign Languages ​​according to the regulations of the Ministry of Education and Training for the type of certificate that includes the exam score (total exam score), candidates must write the score in the "Exam score" box.

Example: 15. Register for exemption from the Foreign Language exam in the recognition of high school graduation or register for admission: (Candidates write down the type of foreign language certificate that meets the requirements according to regulations or clearly state that they are a member of the national team participating in the International Olympic competition in Foreign Languages ​​to be exempted from the exam): TOEFL ITP Test score (If the Certificate has a score, candidates write the score in this box: 450):

Section 16 : Candidates who have taken the high school exam in the previous year, if there are any exams/subjects that are eligible to be reserved according to regulations, candidates who want to reserve the scores of any exam/subject must write the scores of that exam/subject in the corresponding box (Note: For combined exams, candidates are allowed to reserve the results of each component exam if eligible, to reserve the entire score of any combined exam, candidates must write the scores of all component exams of that combined exam). For the reserved exams/subjects, candidates can still register to take the exam (exam or component exam) in Section 14 only if they wish to use the exam results for university and college admission.

Sections 24, 25, 26, 27 on the software : Candidates registering to take the exam to get the results of the University and College entrance exam for Preschool Education must fill in the information in these sections. For Section 24, candidates must declare on the software complete and correct information with evidence according to the regulations and instructions in Appendix XVII to enjoy priority in the University and College entrance exam.
