Obesity screening detects fatty liver and diabetes

The 30-year-old male patient, residing in Bac Ninh, is 177 cm tall, weighs 109 kg, is close to dangerously obese, has never had a medical examination before, and was diagnosed with type 2 diabetes, dyslipidemia, grade 3 fatty liver, and increased liver enzymes.

If the patient continues to be subjective and delay going to the doctor, they may have to be hospitalized for emergency treatment due to acute complications such as ketoacidosis, diabetic hyperosmolar coma, acute pancreatitis, etc. The most serious risk is coma, cerebral edema, and even death.

The patient's fasting blood sugar test results were 15.5 mmol/L, HbA1c quantification was 12.7%, more than twice the average threshold, the Triglyceride cardiovascular health assessment index was 17.6 mmol/L, 10 times the average threshold, the GGT liver enzyme index was 202 U/L, 3 times the normal level, and visceral fat was 164.2 cm2. These are complications due to obesity that was not promptly diagnosed and treated.

Faced with alarming indicators, the patient had to be hospitalized and treated intensively with fluid replacement, insulin injections 4 times a day, combined with oral medications to treat diabetes, dyslipidemia, fatty liver, and other acute symptoms.

After 7 days, the patient felt less tired and the symptoms improved. The patient was discharged from the hospital and continued to be monitored and treated at home with an appropriate weight loss regimen combined with treatment of complications.

In addition to medication, patients are also instructed and advised by doctors on a healthy diet, combined with regular exercise for at least 30 minutes a day.

Two months after treatment, the patient lost 9kg; waist circumference decreased by 10cm, hip circumference decreased by 9cm. In particular, the previous acute alarm indicators disappeared, blood sugar was at 4.79 mmol/l, HbA1c was at 7.16% (nearly reaching the treatment target), Triglyceride was 1.4 mmol/l, liver enzymes and cholesterol were at normal levels.

Patients do not need to inject 4 insulin shots, only maintain 2 groups of pills to control blood sugar, physical condition and health, quality of life is much improved. Patients need to continue to monitor their health periodically to adjust medication combined with a reasonable and scientific diet, exercise, and sleep.

The World Health Organization (WHO) recorded about 1.9 billion overweight and obese people, of which one billion are obese globally, an estimated increase of about 167 million by 2025. Vietnam's obesity rate is not high compared to the region, but the growth rate is the fastest among Southeast Asian countries, at 38%.

Obesity is associated with a risk of many serious diseases. Research shows that people with a BMI over 30 are at increased risk of one or more diseases at the same time, such as knee osteoarthritis, high blood pressure, sleep apnea, gastroesophageal reflux, non-alcoholic fatty liver disease, myocardial infarction, diabetes, major depression, polycystic ovary syndrome, ischemia, congestive heart failure, stroke, and increased risk of cancer.

According to Dr. Vu Thuy Thanh, Center for Weight Control & Obesity Treatment, Tam Anh General Hospital, Hanoi, among patients coming to Tam Anh General Hospital for health check-ups due to fatigue or endocrine diseases, the rate of overweight and obese people accounts for 56%.

At Tam Anh Weight Control and Obesity Treatment Center, the number of people receiving combined treatment for metabolic disorders accounts for 78%, and the number of people coming to the obesity clinic to discover the disease accounts for 77%.

Weight is directly related to body sugar and fat levels, but patients often pay little attention because there are no specific symptoms.

The higher the BMI, the higher the risk of prediabetes and diabetes. Excess fat accumulation in the abdominal area can be a sign of insulin resistance, hindering the process of sugar metabolism, causing blood sugar disorders.

According to Dr. Thanh, it is a mistake to be overweight or obese but not go for a medical examination and check-up. As a result, many people get sick without knowing it, and when the condition gets worse and complications arise, it becomes difficult to treat.

Doctors recommend that overweight and obese people should have regular health check-ups to detect the cause and screen for complications early.

Maintaining a moderate weight and changing to a healthy lifestyle helps prevent diabetes and other complications. Obese people are at high risk of the disease and need to have a balanced diet and exercise plan to control their weight and prevent disease.

Weight loss treatment needs to be performed scientifically, under the guidance of experienced doctors, in coordination with many specialties, to achieve optimal results.
