GĐXH - The patient was admitted to the hospital in a state of exhaustion, thin body, dull abdominal pain, constipation, and an abnormal mass appearing on the abdominal wall in the lower abdomen.


On February 17, information from the 108 Military Central Hospital said that recently, doctors of this unit successfully performed surgery to remove a "giant" mass of feces from a 46-year-old male patient who was diagnosed with partial intestinal obstruction due to a large mass of feces in the colon due to congenital megacolon.

Dr. Ho Huu An, Department of Anorectal and Pelvic Surgery, 108 Central Military Hospital, said that the patient had been sick since childhood, and had been diagnosed with megacolon at the age of 12, but did not receive treatment. For about 3 years now, the patient has seen his abdomen gradually getting bigger; he has difficulty defecating and often has to have an enema to make defecation easier.

The patient was admitted to the hospital in a state of exhaustion; thin body; dull abdominal pain; constipation; and an abnormal mass protruding from the abdominal wall in the lower abdomen. According to the patient, this abnormal mass appeared 3 years ago and gradually grew larger.

Through abdominal examination, doctors discovered an abnormal mass measuring 20x30cm; firm density. On abdominal CT scan: the abnormal mass is a long-term accumulation of feces in the sigmoid colon causing dilation of the entire colon. The largest diameter of the dilated colon loop is 12cm.

The patient was diagnosed with: Low intestinal obstruction due to large fecal mass in the pelvic area/megacolon disease.

The patient was given electrolyte replacement and intravenous nutrition before surgery. During surgery, the entire colon was found to be dilated with a diameter of >10 cm; thick wall; decreased tone. The stool mass was approximately 20 x 30 cm in size in the sigmoid colon.

The patient underwent immediate biopsy to determine the location of the lesion. The sigmoid colon and the stool were removed. After surgery, the patient was stable and discharged from the hospital.

From this case, Dr. Ho Huu An recommends that constipation is a fairly common disease, with many causes (congenital colon dilatation, long colon, anorectal diseases, neurological diseases, etc.). Therefore, patients need to go to specialized facilities for timely diagnosis and treatment to avoid serious illness that affects their health.
