Emergency surgery to save the life of a pregnant woman with severe heart failure

Facing the risk of sudden death due to cardiovascular disease, a 31-week pregnant woman recently had an emergency surgery performed by Hung Vuong Hospital (HCMC) in collaboration with Tam Duc Heart Hospital to save both mother and child.

On November 8, Hung Vuong Hospital announced that it had just saved the life of a mother and child, NTD (33 years old, living in Tien Giang) with a history of 4/4 tricuspid valve regurgitation and pulmonary hypertension 4 years ago. The patient was classified by the doctor as having dangerous complications during pregnancy, but because the pregnancy was unwanted and she wanted a child so much, she decided to keep the pregnancy.

Previously, in mid-October, Ms. D. felt tired, had difficulty breathing, and had a dry cough. She went to Tam Duc Heart Hospital for a check-up and was found to have worsening heart failure, requiring close monitoring of her pregnancy and clinical condition. On October 23, the pregnant woman went to Tam Duc Heart Hospital for a check-up and was found to have severe tricuspid regurgitation at level 4/4, severe pulmonary artery hypertension, right heart chamber dilation, and level 4 heart failure. She was diagnosed with worsening heart failure and was scheduled for a cesarean section when her condition worsened.

The patient was admitted to Hung Vuong Hospital at 6:20 p.m. on October 26 and was monitored for fetal health, had an ultrasound and estimated fetal weight of 1,600-1,700 grams, and was given lung support injections and brain-protective drugs because the fetus was premature. Realizing that the pregnant woman was having difficulty breathing and her heart failure was increasing, on October 27, the Department of Obstetrics and Gynecology, Hung Vuong Hospital held an emergency consultation with Tam Duc Heart Hospital.

After consultation, the doctors assessed the patient's condition as very serious and required an emergency cesarean section, cardiovascular resuscitation first, and agreed to transfer the patient to Tam Duc Heart Hospital to terminate the pregnancy. The surgery was considered a nerve-wracking operation because the pregnant woman could face the risk of sudden heart failure and sudden death, so the doctors from both hospitals had to carefully prepare all treatment options before surgery and concentrate to keep the patient's blood pressure stable during the surgery. Luckily, everything went smoothly.

The baby girl, weighing 1,700 grams, was given respiratory support by doctors at Hung Vuong Hospital right after leaving her mother's body in the operating room of Tam Duc Heart Hospital before being transferred to the Neonatal Department of Hung Vuong Hospital to apply the care regimen for premature babies. After 10 days of surgery and monitoring, the baby's health condition has stabilized. The mother, after being cared for and monitored for acute heart failure at Tam Duc Heart Hospital, was discharged on the morning of November 7.
