According to Dan Tri Electronic Newspaper, the Department of Obstetrics and Gynecology, Thu Cuc TCI International General Hospital has just successfully treated a pregnant woman, Nguyen Thi Quy, and her fetus in a dangerous condition: umbilical cord knot, low fetal heart rate, polyhydramnios, and large fetus.

Pregnancy has many risk factors

In the last months of her pregnancy, Ms. Nguyen Thi Quy, 41 years old, came to Thu Cuc TCI International General Hospital for a routine check-up. As an older mother, this is her fourth pregnancy, through the results of ultrasound and general examination, the doctor predicted that her pregnancy has many risk factors:

– Multiple pregnancies: poor uterine muscle quality, risk of postpartum hemorrhage.

– Polyhydramnios: a condition in which the amount of amniotic fluid exceeds normal levels, which can put great pressure on the uterus and affect the fetal heart.

– Large fetus: the fetus weighs more than average, risk of difficult birth.

– Umbilical cord knot: is an uncommon phenomenon during pregnancy, according to research, the rate of umbilical cord knots accounts for 0.3 - 2.2% of births and increases the fetal mortality rate 4 times higher than normal pregnancies. The level of danger depends on the loose or tight state of the umbilical cord. If the cord is loose, the fetus will be less affected, on the contrary, if the umbilical cord is tight, the fetal circulation will be obstructed, the baby can die in the womb.

– The fetal heart rate fluctuates less: there are many reasons such as the fetus is sleeping so the heart rate slows down and the baby moves less, due to the mother's position, the mother is anemic, fetal distress...

In this condition, pregnant women need to closely monitor with their doctor for timely treatment.

Umbilical cord knot and low fetal heart rate: Dangerous threat to the fetus

According to Dr. Nguyen Van Ha - Head of Obstetrics and Gynecology Department, and Deputy Director of Thu Cuc TC, in a patient with a knotted umbilical cord and low fetal heart rate, an emergency cesarean section is necessary, because every passing minute is a direct threat to the life of the fetus.

The pre-operative preparation process was quick. The mother underwent a comprehensive health check to ensure she was fit for surgery. Modern equipment was mobilized to closely monitor the mother's condition throughout the surgery.

After moments of tension and high concentration from the entire team, the baby boy weighing nearly 4kg was safely delivered from his mother's womb.

Immediately after birth, the baby was examined by a neonatal pediatrician. The results showed that the baby's condition was stable, with a normal heart rate and no other health problems were detected in either mother or baby.

Notes for early detection of umbilical cord knot risk

If during the ultrasound examination during pregnancy, the doctor discovers that the fetus has a knot in the umbilical cord, the mother will be assigned a fetal heart rate test and color Doppler ultrasound to re-evaluate the condition of the fetus.

The most important thing to do is to monitor fetal movements carefully. When the fetus is 26 weeks old, mothers should monitor and count fetal movements every day. When the fetus is sleeping, the movements will decrease or not appear. Each sleep of the fetus usually lasts from 20 to 40 minutes and rarely exceeds 90 minutes. In the awake state, a healthy fetus will make at least 4 movements within 1 hour. If the number of movements is less than 4 in 1 hour, the pregnant woman should lie down and continue to monitor the movements in the next hour or in a period of 2 to 4 hours.

If the fetus moves less than 10 times in 4 hours, you need to quickly go to a medical facility to have the fetus' condition checked using specialized methods.

From week 36 to 40, the mother needs to have a prenatal check-up once a week. The closer the due date is, the more the mother needs to pay attention to the check-up schedule. If any abnormalities are found, go to a medical facility immediately for timely intervention. If the umbilical cord is tightly knotted, an emergency cesarean section may be required to deliver the baby immediately.

If a pregnant woman has a knotted umbilical cord and goes into labor, the fetus needs to be monitored and have its heart rate measured to detect abnormalities in the fetal heart rate. In most cases of a knotted umbilical cord, a cesarean section is recommended because vaginal delivery carries a high risk of fetal distress and loss of fetal heart rate.

Umbilical cord knots can be difficult to detect and the best way to ensure safety is for the mother to monitor fetal movements and attend regular check-ups.
