He said he had a history of prostate enlargement and urinary incontinence for about 10 years but had not received active treatment. Six months ago, he suffered from urinary retention and had to go to the hospital for emergency treatment. The doctor placed a catheter in him to drain urine from his body, however, when the catheter was removed, the urinary retention recurred.

He went to many places for examination, and was diagnosed by a hospital doctor with an enlarged prostate, which was quite large and blocked the bladder neck, which is where urine is expelled, making urination difficult.

However, because he was old, surgery could cause a lot of blood loss and be dangerous, so the doctor only performed surgery to insert a catheter into the bladder through the skin of the lower abdomen to drain the urine that was blocked in the bladder.

However, after 6 months of carrying a catheter with him, he felt very uncomfortable and inconvenient. He had to change the catheter every 10 days, making it difficult to walk, cumbersome, and sometimes urine leaked from the base of the catheter, which was very unhygienic. His family took him to Tam Anh General Hospital in Ho Chi Minh City with the hope that he could be cured completely and not depend on the catheter.

On May 26, Dr. Nguyen Hoang Duc, Head of the Department of Urology, Center for Urology - Nephrology, said that through clinical symptoms, ultrasound and prostate biopsy showed that the patient had benign prostatic hyperplasia. However, the volume of the prostate was up to 240ml, 8 times the allowable level in the elderly, protruding into the bladder, completely blocking the urinary tract.

In this case, the effective treatment is surgical intervention. However, the old methods including open surgery and endoscopic surgery through the urethra both have a huge disadvantage of causing a lot of bleeding, especially for large tumors, which is extremely dangerous for an elderly person.

The doctor decided to perform endoscopic surgery through the urethra, using a specialized instrument with a bipolar electrode to separate the entire tumor from the prostate capsule. The tumor would then fall into the bladder, be crushed and sucked out by a machine. The advantages of this method are less trauma, less blood loss and safety for elderly patients.

The surgery lasted more than 2 hours, the patient lost little blood, and left no incisions. After the surgery, the patient was transferred to the inpatient room, and his health stabilized a day later, he could sit up and eat well. After 2 days, the doctor removed the tube in his abdomen; 6 days after the surgery, the old man could urinate normally and no longer had urinary retention.

Dr. Hoang Duc said that although benign prostatic hyperplasia is not life-threatening, it hinders the flow of urine, greatly affecting the quality of life. The disease is common in men over 50 years old and increases with age.

To prevent urinary retention, if men detect benign prostatic hyperplasia, they should actively visit and monitor the disease with specialists. Up to 95% of patients with benign prostatic hyperplasia live well with medical treatment, only 5% need surgical intervention. In cases where intervention is needed and the tumor is detected when it is small, the treatment will be simple, with few side effects and complications.
