News from Cam Pha Regional General Hospital, where they received patient B.D.T (15 years old, residing in Cam Pha City, Quang Ninh Province) who had a motorbike accident and fell down with a diagnosis of acute hemorrhagic shock due to multiple injuries, chest, abdominal, cranial, and maxillofacial injuries... The patient was immediately resuscitated.

CT scan revealed that the patient had subdural hemorrhage with parenchymal contusion, but diffuse axonal injury was not ruled out.

Along with that, the patient suffered multiple other injuries, fracture of the left mandibular ramus, edema with soft tissue effusion around the left ear; fracture of the left clavicle and fractures of the left ribs 1 and 6; contusion of lung parenchyma, contusion of the left chest wall and neck muscles; small left pneumothorax; spleen injury, left kidney injury...

The on-duty team consulted and agreed to perform emergency surgery on the patient. At the same time, the on-duty leader reported to the Quang Ninh General Hospital to support the abdominal surgery team and provide 4 units of blood to supplement the blood supply for the surgery.

Despite receiving two units of blood and a large amount of intravenous fluids at the emergency room, the patient's blood pressure continued to drop. The patient was then immediately transferred to the operating room.

During the surgery, the surgical team discovered that the patient's abdomen was filled with blood, with a piece of spleen (5x6cm in size) crushed and floating freely in the abdomen from the splenic stalk, and a tear in the left liver...

Immediately afterwards, another surgical team arrived and continued to examine, finding the injuries very severe, so they intervened by cutting the spleen, stopping the bleeding in the splenic pedicle; suturing the left liver to stop bleeding; and draining the abdomen. The surgery was transfused with 10 units of blood and 6 units of plasma. After the surgery, the patient was transferred to the Intensive Care - Anti-Poison - Artificial Kidney Department for further treatment.

However, after the patient regained consciousness, the doctors discovered signs of weak left radial pulse, suspected of damage to the blood vessels and nerves of the left arm due to a fracture of the left clavicle. After monitoring and consultation, the doctors decided to continue surgery to treat the left subclavian artery and vein injury, while simultaneously performing a left clavicle fusion and bilateral jaw fixation.

The doctors took the saphenous vein from the right thigh and reversed the connection to restore the left subclavian artery, and sutured the wound on the side of the subclavian vein. At the same time, the surgical team combined the left clavicle, treated the tendon injuries, and performed bilateral jaw fixation surgery. After surgery, the patient continued to be treated at the Department of Intensive Care - Anti-Poison - Artificial Kidney.

Currently, patient B.D.T is stable and alert. It is expected that the jaw fixation will be removed after 3 weeks and the arm nerve injury will be treated after 3 months.
