On March 7, Dr. Nguyen Minh Tien - Deputy Director of the City Children's Hospital (HCMC) - said that he had just saved the life of a 1-year-old boy named NVTP (living in Hau Giang ) who had a rare tracheal rupture and multiple broken ribs after falling from bed to the floor.
Medical history revealed that the child was sitting and playing on the bed when he fell to the floor in a supine position. After the fall, the child cried but was alert, did not vomit, and did not have convulsions. On the second day, the family saw that the child had swelling in the neck and chest, so they took him to the local hospital for examination and admission.
At the hospital, the doctor noted that the child was lethargic, had purple lips, difficulty breathing, SpO2 85% and was intubated, ventilated, and had a CT scan of the head - chest - abdomen.
The X-ray results showed that the child had subcutaneous emphysema in the abdomen, hips, back, chest, and neck on both sides, collapsed upper lobes of the lungs on both sides and the middle lobe of the right lung, ruptured trachea, pneumothorax, and fractures of ribs 8, 9, and 10...
Baby P. was transferred to the City Children's Hospital and was consulted by respiratory, ENT, surgical resuscitation, and anesthesia specialists. The diagnosis was: tracheal rupture, rib fractures 8, 9, 10 (right). The doctors agreed to perform open chest surgery to reconstruct the trachea, combined with bronchoscopy during surgery.
After surgery, the patient was transferred to the surgical intensive care unit for further treatment with respiratory support, antibiotics, intravenous fluids, sedatives, and electrolyte and acid-base adjustment.
After nearly 2 weeks of treatment, the child's condition improved, the mediastinal and subcutaneous emphysema gradually disappeared, the ventilator was removed, and the pleural drainage tube was removed. The child was alert and breathing fresh air.
Doctor Minh Tien said that the case of patient P. is a rare case of tracheal rupture in children.
Through this case, doctors note that there should always be someone to look after children under 3 years old, because at this age children often explore the world around them such as crawling, touching strange objects, peeling off strange objects and putting them in their mouths... leading to unfortunate accidents (burns, electric shock, poisoning due to drinking or eating chemicals, medicine by mistake, injuries due to falling...).
