The surgical technique of placing cortical surface electrodes to map the epileptogenic zone contributes to solving difficult cases of complex epilepsy, thereby improving the quality of treatment and improving the lives of patients.

Doctors at the National Children's Hospital have successfully performed surgery to place surface electrodes on the cerebral cortex to map the epileptic region for a 6-year-old patient with NNM.

Dr. Le Nam Thang, Head of the Department of Neurosurgery, Neurology Center, National Children's Hospital, said that this is the first case of complex epilepsy in which doctors at the National Children's Hospital performed surgery using the technique of placing electrodes on the surface of the cerebral cortex to map the epileptic region, with the support of 7 experts from the US.

To perform surgery on this child, doctors had to place electrodes 36-48 hours in advance for monitoring. Then, after more than 4 hours of surgery, Dr. Le Nam Thang and his colleagues, with the support of American experts, completely removed the damaged area that caused the epileptic seizures in the child patient NNM.

NNM patient is gradually recovering after surgery to treat epilepsy.

"Before the surgery, the child had 50-100 seizures per day and always had to take sleeping pills. After the surgery, the patient was almost awake, had no seizures and was completely paralyzed. The first surgery we have performed so far has been a complete success," shared Dr. Le Nam Thang.

In addition, to bring this technique to Vietnam, doctors at the National Children's Hospital have had nearly 2 years of preparation. Dr. Le Nam Thang and a number of resuscitation doctors have gone to the US to study and receive the transfer of this technique. At the end of September, 3 more doctors at the National Children's Hospital will continue to study for 3 months at Alabama Hospital, University of Alabama, USA.

Doctors at the National Children's Hospital are preparing to perform another surgery to treat epilepsy using the technique of placing electrodes on the surface of the cerebral cortex.

The advancement in performing this surgical technique is to solve difficult cases of complex epilepsy, thereby improving the quality of treatment and improving the lives of patients and their families. However, this is a difficult and expensive technique. In the US, the cost of surgery alone is 150,000 USD, not including the cost of treatment and medication.

"In the future, we hope to have policies for surgeries like this to help unfortunate children with epilepsy have the opportunity to return to normal life," Dr. Le Nam Thang expressed.

Professor Brandon Rocque, Alabama Children's Hospital, USA, said that surgery to place electrodes on the surface of the cerebral cortex to map the epileptic region is indicated for patients with signs such as: drug-resistant epilepsy, unclear epileptic region, epileptic region near the language and motor function area.

This is a complex technique that involves many anatomical functional areas, as well as the patient's post-operative psychology. This technique has been performed in the US and Europe for about the past 5 years and was first performed at the National Children's Hospital. This is also the first medical facility in Southeast Asia to receive this technique transfer.
