The Ministry of Health has just issued guidelines for the arrangement and reorganization of medical facilities at administrative units at all levels, implementing a two-level local government organization model.
According to this instruction, the existing Health Department under the People's Committee at district, county and town levels will be dissolved, end its operations, and transfer the functions and tasks of advising on state management of the former Health Department to the Department of Culture - Society and the Public Administration Center under the People's Committee at the grassroots level (commune and ward level) after being reorganized to continue implementation.
The tasks and powers of the People's Committee at the grassroots level (commune and ward level) in state management of health include: preventive medicine; medical examination, treatment, rehabilitation; traditional medicine and pharmacy; reproductive health; medical equipment; pharmaceuticals; cosmetics; food safety; health insurance; population; social protection; child protection and care; prevention and control of social evils.
Directors of the Department of Health of provinces and cities are responsible for supporting human resources, directing and providing professional guidance to help the People's Committees at the grassroots level perform their tasks and powers of state management of health in communes and wards.
For existing district, county, town and city general hospitals : Basically maintain and convert into regional general hospitals, which are public service units under the Department of Health of the province or city after reorganization, to organize the provision of medical examination and treatment services for people in the inter-commune and ward areas, regardless of administrative boundaries.
For District, County, Town and City Health Centers : Basically maintain and convert these Health Centers into Regional Health Centers, which are public service units directly under the Department of Health of the province or city after the arrangement, to perform the functions, tasks and powers according to Circular No. 32/2024/TT-BYT dated November 15, 2024 of the Ministry of Health guiding the functions, tasks, powers and organizational structure of District, County, Town and City Health Centers under provinces, cities and cities under centrally run cities and continue to support human resources, professional and technical guidance, ensure medicines, supplies and medical equipment for commune and ward health stations after the arrangement.
Based on the review and assessment of the scope of operations and operational efficiency of existing district, town, and city health centers, the Director of the provincial and municipal health department shall advise and develop a project and submit it to the People's Committee of the province or city to decide to maintain all or dissolve, reorganize, or merge a number of district, town, and city health centers in the area.

Mobilize and supplement human resources from provincial-level medical facilities to grassroots-level medical stations
Maintain and preserve existing regional polyclinics, commune, ward and town health stations and transfer them to the People's Committees at the grassroots level (new communes and wards) for management. Ensure that each new commune and ward after the reorganization has at least one health station. Depending on the population size and geographical characteristics of each new commune and ward, maintain or reorganize and establish additional health stations managed by the People's Committees at the grassroots level to meet the primary health care needs of people in that area.
The functions, tasks and powers of commune and ward health stations continue to be implemented according to the provisions of Circular No. 33/2015/TT-BYT dated October 27, 2015 of the Ministry of Health.
The human resources of the new commune and ward health stations are arranged on the basis of receiving officials from the previous commune and ward health stations and are mobilized and supplemented from provincial-level health facilities, from the previous district, county, town and city health centers, to ensure that each new commune and ward health station has at least 2 doctors or more.
The Ministry of Health said that it will issue a Circular guiding the functions, tasks, powers and human resource structure of commune and ward health stations to replace current circulars.
Directors of the Department of Health of provinces and cities are responsible for directing organizations and units under and directly under the Department of Health to support grassroots health stations in terms of human resources, professional and technical guidance, ensuring adequate medicine, equipment, medical supplies and maintaining health insurance medical examination and treatment contracts at health stations to ensure the efficiency, effectiveness and efficiency of the operations of grassroots health stations, without interruption in the provision of health protection and care services to people in the new communes and wards.

Plan to arrange and reorganize medical facilities at provincial and municipal levels directly under the Central Government
According to this guidance, the following provinces and cities will arrange and merge to establish their own Department of Health based on the merger of the existing Department of Health of the current provinces and cities. The Ministry of Health will issue a Circular guiding the functions, tasks, powers and human resource structure of the Department of Health of the provinces and cities to replace the current circular.
Also according to this instruction, the Directors of the Department of Health of the following provinces and cities shall arrange to advise and submit to the People's Committees of the provinces and cities to decide to maintain, dissolve or reorganize public service units under the Department of Health according to the principle: ensuring the maintenance of the provision of basic and essential services in the fields under the state management of the health sector in the locality.
At the same time, basically maintain and keep the existing medical examination and treatment facilities (general, specialized, regional) and social protection and social assistance facilities, which are public service units under the Department of Health after the arrangement, to maintain the provision of basic or specialized medical examination and treatment services and social assistance services, social work services.
For other public service units under the Department of Health (Center for Disease Control, Medical Examination Center, Forensic Center, Drug, Cosmetic, Food Testing Center, etc.), the Director of the Department of Health shall develop a project to submit to the People's Committees of provinces and cities on the merger of public service units with the same name, functions, tasks, and powers to establish a new public service unit.
Depending on population size, geographical characteristics, and scope of operations, each of these new units may have multiple operating bases, but must clearly define tasks, powers, scope of operations, and coordination mechanisms.
The Ministry of Health recommends that the People's Committees of provinces and cities base on these instructions and local practical conditions to implement, ensuring the effectiveness and efficiency of the operations of medical facilities after rearrangement and reorganization.
In the process of arranging and reorganizing medical facilities in the area, if there are any difficulties or problems, the People's Committees of provinces and cities will discuss and coordinate with the Ministry of Health to continue to guide implementation in accordance with regulations.
