Detecting metastatic bladder cancer after self-treatment at home

Taking a medical history, Mr. T. said that in the past two years, urination had become a nightmare for him because of the terrible pain, and sometimes he even passed blood clots. He went to many places for examination, had many tests and cystoscopy, but the diagnosis was all chronic cystitis. He was prescribed oral medication, bladder injections, and monthly follow-up. After a year of treatment, he became discouraged, stopped follow-up visits, and self-treated at home with folk remedies.

Recently, he continued to have frequent hematuria, so he went to a hospital in Ho Chi Minh City for examination. He had a bladder biopsy and found a malignant tumor in his bladder. His family took him to Tam Anh General Hospital in Ho Chi Minh City for treatment.

On October 11, Dr. Nguyen Hoang Duc, Head of the Department of Urology, Tam Anh General Hospital, Ho Chi Minh City, said that through examination and whole-body magnetic resonance imaging (MRI), Mr. T. had many malignant tumors of different sizes scattered throughout his bladder. More seriously, the malignant tumors had invaded deep into the bladder muscle layer and metastasized to some lymph nodes in the pelvic area.

According to Dr. Duc, in the case of early-stage bladder cancer, it can be treated by surgery to remove the entire bladder, then create a new bladder from a section of the small intestine, and then connect the urethra and two ureters. With this method, the patient can urinate naturally after surgery.

"However, in Mr. T's case, the cancer cells had invaded deep into the muscle and had metastasized to the bladder neck and urethra, so this option could not be applied. The optimal option is to surgically remove the entire bladder, scrape out all metastatic lymph nodes, and then use the small intestine to divert urine from the kidney directly out of the body through a small hole in the abdominal wall," the doctor shared.

The surgical team made four small holes of 0.5-1cm in Mr. T's abdomen to insert the laparoscopic surgical instruments inside. The team then wore 3D glasses to perform the surgery with the 3D endoscopy system.

The surgery was completed after about 5 hours. Seven days after the surgery, Mr. T. recovered well, had no pain, could walk normally, and was discharged from the hospital.

Early signs of bladder cancer are easily confused

According to Dr. Nguyen Hoang Duc, the cause of bladder cancer is currently unknown. However, people who smoke, have been infected with parasites, are frequently exposed to toxic chemicals or radiation, etc. have a higher risk of developing the disease.

"Bladder cancer is difficult to detect because the early signs are easily confused. Therefore, doctors recommend that patients pay attention to reputable medical facilities for early examination when they have symptoms of recurrent hematuria (the most common symptom); painful urination, frequent urination; fatigue, loss of appetite, rapid weight loss for unknown reasons...", the doctor recommends.
