The first case occurred at Kim An Beauty Salon Company Limited located at 148C Tran Quang Khai, Tan Dinh Ward, District 1, Ho Chi Minh City.

Kim An Beauty Salon Company Limited at 148C Tran Quang Khai, Tan Dinh Ward, District 1, Ho Chi Minh City was granted a Business Registration Certificate by the Department of Planning and Investment of Ho Chi Minh City on July 10, 2024, with Mr. LNKN as Director, with the business line of "Operations of general, specialized and dental clinics; haircutting, shampooing; skin care; tattooing, spraying, embroidering on the skin (not using injectable anesthetics..."). The Department of Health has not yet granted a license for medical examination and treatment activities.

According to information from the Ho Chi Minh City Department of Health, on the afternoon of October 22, 2024, the patient came to this facility and was received by BTL staff (8th grade, no medical degree), weighed and measured the patient's fat condition, advised the patient to undergo liposuction surgery for 30 million (the patient had paid 17 million), then ordered a rapid blood sugar test, rapid HIV test, Quick test urine test and was tested and read the test results by LTTN staff (also no medical degree).

After that, the patient underwent liposuction surgery on the 5th floor performed by Mr. D. The surgery started at 6:00 p.m. and ended around 8:30 p.m. the same day. After that, when the patient was found to be lethargic and slow to respond, his family called the Tan Dinh Ward Police.

When the Inspection Team asked Mr. LNKN to provide the professional qualifications and practice licenses of Mr. D. and the team performing liposuction on the patient, Mr. LNKN could only provide the phone number. When calling, Mr. D. did not answer the phone.

Mr. LNKN said that this company has submitted documents to the Department of Health requesting a License to operate a Cosmetic Clinic under Kim An Cosmetic Hospital Company Limited, with Dr. TTNH in charge of technical expertise, currently waiting for the Department of Health's approval.

District 1 Health Department has drawn up 3 administrative violation records No. 08/BB-VPHC, No. 09/BB-VPHC and No. 10/BB-VPHC against: (1) For Kim An Beauty Salon Company Limited, of which Mr. LNKN is the Director, the act: Providing medical examination and treatment services without a business license, the additional penalty is to suspend the facility's operations for a period of 18 months;

For Mr. BTL and Ms. LTTN, the behavior: Providing medical examination and treatment without being granted a practice certificate. At the same time, at the time of the inspection, the General Investigation Team, District 1 Police also collected evidence, documents,... and worked with related individuals.

On the other hand, the Delegation discovered that the facility had a customer profile listed under the name FA + Beauty Salon. According to Ms. HKD - Mr. LNKN's wife, this facility was transferred from FA + Beauty Salon Company Limited, address: 190 Nguyen Van Thu, Ward Da Kao, District 1, Ho Chi Minh City.

On April 9, 2024, the Department of Health Inspectorate issued a Decision to impose an administrative penalty on FA + Beauty Salon Company Limited, of which Ms. HKD is the Director, for the act of providing medical examination and treatment services during the period of suspension of operation. The additional penalty is to suspend the facility's operations for a period of 18 months.

Up to now, FA + Beauty Salon Company Limited has not yet paid the fine even though the Health Department Inspectorate has reminded them to pay the fine 3 times.

The Department of Health requested Mr. D. and the team that performed liposuction on the patient to work at the People's Committee of District 1 to clarify the violations. The Department of Health Inspectorate continued to support the District 1 Health Department to clarify the violations of the individuals involved. It was requested that the District 1 Health Department continue to advise the District 1 People's Committee to issue a Decision on administrative sanctions in accordance with regulations, and report the results to the Department of Health Inspectorate.

The second case was recorded through a quick report from Thu Duc Regional General Hospital about a cosmetic surgery case on October 21, 2024 at the MIN Beauty Academy facility located at: 50C Street 385, Tang Nhon Phu A Ward, Thu Duc City.

Conducting a quick information lookup on the application "Look up information on medical and pharmaceutical practice in Ho Chi Minh City" recorded that the above address has not been licensed by the Department of Health to conduct medical examination and treatment (illegal operation). The Department of Health Inspectorate coordinated with the Thu Duc City Health Department, the Ward People's Committee, and the Police of Tang Nhon Phu A Ward, Thu Duc City to inspect the facility.

At the time of inspection, the facility was closed, no one inside, no sign outside, and a sign reading "MIN beauty" inside the locked door.

Through information review, at this address there is a certificate of Min Beauty Academy Business Household, business code: 41Y8000358 issued by the Department of Economics - Planning and Investment - Thu Duc City People's Committee to Ms. D.TTH; Business line "Haircutting, hairdressing, shampooing, Sauna services and similar health improvement services (except sports activities...").

The patient informed through the Facebook page of the facility "MIN Beauty Academy" at 50C Street 385, Ward Tang Nhon Phu A, Thu Duc City and an acquaintance's introduction.

On October 21, 2024, about 3 hours before being admitted to the hospital, the patient went to MIN Beauty Academy to have rhinoplasty surgery performed by Ms. D.TTH.

After being injected with anesthesia and having surgery, the patient had difficulty breathing and was transferred to Le Van Viet Hospital for emergency care and then transferred to Thu Duc Regional General Hospital.

The Health Department Inspector invited the business owner to work, however, Ms. D.TTH did not come to work as scheduled, showing signs of avoiding state management agencies.

Faced with the reality of illegal cosmetic practice, causing serious consequences to people's health, signs of assault, avoidance, and escape when causing complications for customers at "underground" establishments, the Department of Health will transfer the case file to the City Police to investigate, clarify and strictly handle according to the law.

Guidance for medical examination and treatment facilities to complete documents and procedures for granting and adjusting operating licenses

The Department of Medical Examination and Treatment Management has just organized an online training course to guide on documents and procedures for adjusting medical examination and treatment licenses for medical examination and treatment facilities under the Ministry of Health, health departments of ministries, branches, university hospitals and health departments of provinces and centrally run cities.

Dr. Ha Anh Duc, Director of the Department of Medical Examination and Treatment Management, said that preparing documents and procedures to request adjustment of medical examination and treatment operation licenses in accordance with the provisions of the Law on Medical Examination and Treatment No. 15/2023/QH15 dated January 9, 2023 and Decree No. 96/2023/ND-CP dated December 30, 2023 of the Government detailing a number of articles of the Law on Medical Examination and Treatment is an important task, deciding on the adjustment of medical examination and treatment licenses and shortening the time to handle this administrative procedure.

Recently, a number of public medical examination and treatment facilities have prepared documents requesting adjustments to their operating licenses when changing their scale of operations, scope of professional activities, or adding technical categories.

However, the dossier is still incomplete: such as lacking evidence of professional activity data as a basis for the proposal to adjust the increase in hospital bed size, establish new or separate specialized departments; human resources do not meet the proposed adjustment; do not demonstrate meeting the requirements on floor space per hospital bed... affecting the appraisal and licensing process.

Through training, management agencies and medical examination and treatment facilities will better understand and implement the preparation of records, procedures, and processes for granting and adjusting operating licenses in accordance with legal regulations. At the same time, it will be a forum for the Department of Medical Examination and Treatment to answer, receive, promptly resolve problems and difficulties, and amend and supplement regulations accordingly.

Also according to Dr. Ha Anh Duc, the Ministry of Health is asking for opinions from localities on the draft Circular on decentralization of administrative procedures for granting operating licenses, and requests that the Departments of Health of provinces and cities give their opinions on the draft circular to be completed.

Today, October 25, 2024, the Department of Medical Examination and Treatment Management continued to meet with units on technical expertise ranking and decentralization of administrative procedure settlement in the field of medical examination and treatment under the management of the Ministry of Health.

Update on diagnosis and treatment of acute ischemic stroke

Acute ischemic stroke (cerebral ischemia) is the cause of more than 80% of stroke cases with the number of patients increasing.

To improve the effectiveness of emergency care, treatment and optimize existing equipment at hospitals, 108 Central Military Hospital organized a scientific workshop "Update on diagnosis and treatment of acute cerebral infarction".

Major General, Associate Professor, Dr. Le Van Truong, Deputy Director of the 108 Central Military Hospital, emphasized that in the context of the current military hospital system, due to different functions, tasks, and actual conditions, the development of the stroke specialty and stroke intervention is also uneven.

That leads to the situation where when a patient randomly appears at any hospital, access to treatment, quality and treatment results are not guaranteed.

To limit the above shortcomings, it is urgent to organize a specialized linkage system within each hospital and between hospitals, which is also the meaning of organizing the Scientific Conference.

According to Associate Professor, Dr. Le Van Truong, in order not to miss the opportunity to treat patients with cerebrovascular disease and stroke, it is necessary to: Build a stroke diagnosis and treatment system according to the practice of each hospital; equip equipment for cerebrovascular imaging diagnosis;

At the same time, train doctors and technicians to meet the need for 24/7 equipment use; strengthen teamwork between hospitals and outpatients; organize specialized conferences and seminars to regularly update and exchange knowledge and experience.

Regarding the method of cerebral revascularization in the treatment of stroke, Senior Lieutenant Colonel, Dr. Nguyen Trong Tuyen, Head of the Department of Neurological Intervention, Institute of Neurology, 108 Central Military Hospital, said that currently, there are many techniques for removing cerebral artery thrombosis such as direct thrombosis with a wide-bore suction tube (ADAPT), thrombosis with stents, balloon angioplasty and stent placement.

Some new techniques have also been deployed such as thrombectomy using Solumbra, SAVE, ARST, BADDASS techniques...

Cerebral revascularization techniques using mechanical devices are increasingly developing with new techniques and new devices, so it is necessary to individualize each patient to choose the appropriate treatment method.

It is known that Vietnam is one of the countries with the highest stroke rate, with about 200,000 people suffering from stroke each year.

This is the second most common cause of death in Vietnam. Among stroke survivors, the rate of disability due to stroke is high.

The American Heart and Stroke Association has made dietary recommendations to prevent stroke, such as eating plenty of fruits and vegetables; choosing whole-grain, high-fiber foods; reducing meat in meals so that at least 50% of the diet is fruits and vegetables; 25% is high-fiber grains; eating fish at least twice a week and choosing fish rich in omega 3 such as salmon or tuna.

Also, limit cholesterol, saturated fat, and trans fat; choose lean meats and poultry and avoid saturated or trans fat when preparing food; avoid beverages and foods with added sugar; and choose and prepare foods with spices and seasoning mixes that limit salt.

It is important to limit alcohol as much as possible because it can interact adversely with some medications that patients are taking to prevent stroke recurrence (e.g. warfarin). Alcohol abuse will cause high blood pressure, increasing the risk of stroke recurrence.
