"The hospital has tested and detected two patients with rare malignant syphilis, on the basis of HIV patients, homosexual relationships." The above information was reported by Dr. CK.II Nguyen Thi Thanh Tho, Deputy Head of Clinical Department 3, Ho Chi Minh City Dermatology Hospital, at the 19th Annual Scientific Conference of the Ho Chi Minh City Dermatology Association on the afternoon of May 21.

HIV, homosexuality and syphilis

The first case is a 19-year-old patient living in Dong Thap. Accordingly, the patient saw many ulcers on his skin, oozing pus, and his chin with swollen joints. The patient was examined in the province, treated with antibiotics, anti-inflammatory drugs and pain relievers but did not improve. The old ulcers were black and crusted, many new ulcers appeared, making the patient worried and went to the Dermatology Hospital for examination.

This patient had a history of homosexuality and had more than two sexual partners. The patient was diagnosed with HIV 5 years ago and is on ARV treatment.

Abdominal ulcer of patient with malignant syphilis is stable after treatment

Suspecting that the patient had syphilis on top of existing HIV, the doctor instructed the patient to undergo screening tests for all sexually transmitted diseases. The results showed that the patient had malignant syphilis and was treated according to the regimen.

Similarly, a 27-year-old patient with a history of HIV infection and homosexuality came to the clinic with a skin ulcer near the anus for 1 month. The patient's entire body had many deep ulcers, oozing foul-smelling fluid, no pain... There were a few dry ulcers, the surface covered with a thick, dark brown crust.

The patient was tested and diagnosed with malignant syphilis. Evaluation 2 weeks and 6 months after treatment according to the regimen showed that the skin lesions had almost completely healed.

Short incubation period

Doctor Nguyen Thi Thanh Tho said that malignant syphilis has a short incubation period, starting with systemic symptoms such as fever, chills, muscle aches, joint pain, etc. Skin manifestations progress from nodules and pustules to ulcers, oozing ulcers, forming a thick crusty layer on the surface like a shell, brown or black in color.

"Diagnosis is based on patient examination combined with paraclinical testing. If not diagnosed and treated early, the disease can progress throughout the body, affecting the cardiovascular system, central nervous system, vision, hearing, musculoskeletal system, digestion, kidneys and urinary system...", Dr. Thanh Tho informed and affirmed that these cases of malignant syphilis improved dramatically after treatment with specific antibiotics.

Dr. Thanh Tho noted that malignant syphilis is very rare, can cause complications in organ systems and is life-threatening. This disease should be considered when receiving patients with HIV, a history of homosexual intercourse, ulcerative or necrotic lesions, and may be accompanied by systemic symptoms. The prognosis is good when treated early, with the right and sufficient dose of antibiotics according to hospital protocol.

According to Dr. Thanh Tho, syphilis is no longer strange to Vietnamese people and there is an effective treatment regimen. People infected with syphilis may or may not have symptoms on the skin and the whole body. Of which, malignant syphilis is a rare serious form of secondary syphilis.
