Female patient with maxillary sinusitis causing life-threatening complications

The patient is PTTN (23 years old, residing in Thanh Tri District, Soc Trang), with a history of diabetes since the age of 7. At the age of 18, Ms. N. had end-stage chronic kidney failure, had to undergo regular dialysis, and had many diseases such as high blood pressure, mononodular goiter, and chronic sinusitis.

Recently, the sinusitis became worse, causing headaches, dizziness, stuffy nose, phlegm, and runny nose. Ms. N. took medicine but it did not work, so her family took her to Thanh Vu Medic Bac Lieu General Hospital for treatment.

Doctor Nguyen Hong Tru, Head of the ENT Clinic, Thanh Vu Medic Bac Lieu General Hospital, said that when receiving the patient, the doctor ordered paraclinical tests. The CT-Scan results of the head showed that the patient had left maxillary sinusitis, spreading and causing a perforated septum to invade the right nasal cavity, which was very dangerous.

The patient is young but has many underlying diseases and must undergo regular dialysis, so choosing a surgical method is very difficult, because there is a risk of complications such as blood clots or massive bleeding.

After consultation, the doctors agreed on the treatment method of dialysis one day before surgery. During the surgery, the doctor adjusted the dosage of anticoagulants to prevent bleeding. After that, the left maxillary sinus was opened, and a lot of pus and green fungus clumped together were discovered. The doctor immediately flushed the maxillary sinus, disinfected and cleaned the sinus area.

The surgery was successful, the patient is recovering well and is expected to be discharged in the next few days.

Through this, Dr. Nguyen Hong Tru recommends that when the maxillary sinus is infected, the fungus invades the eye socket, causing the eyes to gradually become blurred and can cause permanent blindness. The fungus can cause meningitis and encephalitis if it invades the skull. In addition, if the fungus invades large blood vessels, it can cause massive bleeding, which is life-threatening. Therefore, when abnormal symptoms appear in the nose and sinus area, the patient needs to go to the hospital immediately for timely examination and treatment to avoid future complications.
