What should I do if I lose my social security book and leaflet?

Ms. Hoai Anh checked the separate sheet issued by the Social Insurance agency and found that the time of participating in Social Insurance was lost from October 2018 to May 2019.

Specifically, her social insurance participation period at the first company was from October 2018 to December 2019. This period is displayed in the social insurance application - VssID. However, the separate sheet when closing the social insurance book for her only recorded the period from June 2019 to December 2019.

Unlike Ms. Hoai Anh, Mr. Hy withdrew his social insurance in 2022, so his social insurance book was revoked. After that, Hy went back to work and participated in social insurance. He currently wants to re-issue his social insurance book and has completed the procedure on VssID but was rejected.

The procedure for reissuing a social insurance book is quite simple (Illustration: Son Nguyen).

According to Vietnam Social Security, in cases of lost books or separate sheets, employees can carry out procedures to issue lost books in 2 ways.

Firstly, employees can carry out the procedure for issuing lost social insurance books and leaflets on the social insurance application number - VssID in 4 steps.

Step 1: Log in to your VssID account using your social security number and password issued by the social security agency.

Step 2: On the Personal Management page, select Public Services.

Step 3: Select Reissue social insurance book without changing information.

Step 4: Select to receive results via postal service and enter a specific address.

Second, employees can carry out procedures to issue lost books and separate sheets at the Social Insurance agency.

According to Consolidated Document No. 2525 dated August 15, 2023 of Vietnam Social Security, to complete the procedure for reissuing a lost social insurance book, employees only need to fill out a declaration of participation and adjust social insurance and health insurance information (Form TK1-TS).

If the employee is working, submit the above declaration to the unit where he/she is working or to the Social Insurance agency.

If the person applying for the procedure is reserving the social insurance payment period or has been settled to receive pension or social insurance benefits and requests to reissue or adjust the content on the social insurance book, he/she must submit it to the social insurance agency nationwide.

According to Vietnam Social Security, if submitting paper documents, employees can go to the Social Security agency to submit documents directly, or send documents via postal service to the one-stop department of the district/town Social Security.

In addition, the procedure for reissuing social insurance books can now be done online on the National Public Service Portal. Employees can access the portal at https://dichvucong.gov.vn to submit their applications online.
