According to the report of Tam Duong District Medical Center (Lai Chau), at around 12 noon on April 10, Mr. Sung A Pao (46 years old, residing in Chu Va 8 village, Son Binh commune, Tam Duong district) organized lunch with his son Sung A Duong (21 years old) and daughter-in-law Giang Thi Cau (18 years old) at the farmhouse, after work. The lunch consisted of mushroom soup and white rice.

At around 10pm, Ms. Cau had symptoms of stomachache, vomited 9 times and had diarrhea many times. At around 1am on April 11, Mr. Duong also had a headache, stomachache, vomited 8 times and had diarrhea many times and at 4am the same day, Mr. Pao also had similar symptoms.

At around 6 p.m. on April 11, the patients were taken to Tam Duong District Medical Center for examination and treatment. Mr. Pao's health was stable, alert, and he did not vomit; the remaining two patients were transferred to Lai Chau Provincial General Hospital for treatment.

At Lai Chau General Hospital, Ms. Cau was diagnosed with acute liver failure, suspected to be caused by mushroom poisoning, gastrointestinal bleeding and was treated according to the protocol. Mr. Duong was also diagnosed with the same. The two patients continued to worsen and were transferred to Hanoi for treatment.

The leader of Son Binh Commune People's Committee said that Ms. Cau had died and Mr. Duong was in a coma and in critical condition.

Through verification, the authorities determined that Mr. Pao's family used 3 large, umbrella-shaped, dark brown mushrooms to cook soup. These mushrooms were picked by Mr. Pao on the field.

The Food Safety and Hygiene Department (Lai Chau Provincial Department of Health) has taken samples to send to the National Institute for Food Safety and Hygiene Testing for testing. At the same time, Lai Chau authorities have also established two investigation teams to verify at Tam Duong District Medical Center; at Lai Chau Provincial General Hospital and the place where the suspected poisoning incident occurred.
