Ms. TTK (68 years old, living in Cu Chi, Ho Chi Minh City) has had pain in her left flank for many years, recently the condition has gotten worse with painful urination.
On December 11, Master - Specialist Doctor 2 Nguyen Vinh Binh, Head of the Department of Urology, Xuyen A General Hospital, Ho Chi Minh City, said that patient K. was discovered by doctors to have grade 4 hydronephrosis when examined at the Elderly Health Care program organized by the locality in conjunction with the hospital.
Taking a medical history, Ms. K. said that 10 years ago, she was diagnosed with left ureteral stenosis and was treated at another hospital. Recently, she often had painful urination accompanied by pain in the left flank. After being accidentally diagnosed with the disease at the Elderly Health Care program, Ms. K. decided to go to the hospital for treatment.
At the Department of Urology, through examination and necessary tests, the patient was diagnosed with simultaneous ureteral stenosis at two different segments (the intravesical ureter and the left dorsal ureter). This is the cause of grade 4 hydronephrosis of the left kidney.

The surgery was performed immediately after that. During the more than 8 hours of surgery, the doctors worked together to solve each problem one by one. First, they determined the location of the 2 narrow segments on the fluoroscopy screen, then they performed an open surgery to insert the narrow pelvic ureter into the bladder using the Lich-Gregoir method, and finally they performed a retroperitoneal laparoscopic surgery to create a dorsal ureteral segment with an end-to-end anastomosis.
With close coordination between the doctors, the surgery went smoothly. The patient recovered well after surgery and returned to normal activities in a short time, even though the patient is 68 years old this year.
According to Dr. Binh, if hydronephrosis is examined and detected early, it will have a better treatment and recovery direction. Therefore, the doctor recommends that people pay attention to regular general health check-ups. This is very necessary to detect diseases early, even if they do not show symptoms, thereby having a timely treatment plan to preserve kidney function before it progresses to chronic kidney failure.
