According to information from the Ministry of Health, on May 11, the Department of Preventive Medicine received Report No. 599/KSBT-PCBTN from the Thanh Hoa Provincial Center for Disease Control dated May 10, 2023 on the vaccination incident at Thang Binh Medical Station, Nong Cong District.
According to the Center's report, on the morning of May 9, the Health Station vaccinated 4/6 children aged 2 to 4 months with the first dose of Hexaxim vaccine, which expired in March 2023.
To carry out the management of vaccine use according to the provisions of Decree No. 104/2016/ND-CP dated July 1, 2016 of the Government regulating vaccination activities and regulations of the Ministry of Health on vaccination, the Department of Preventive Medicine requests the Department of Health to organize an investigation, hold a meeting of the Professional Advisory Council to assess the cause of serious adverse events during the use of vaccines to evaluate and conclude on the above case, deploy incident handling activities and report according to regulations;
Review and clarify responsibilities of violating units and individuals and seriously rectify the implementation of vaccination procedures locally, not allowing errors to occur during the vaccination process.
Review the entire vaccination process to rectify vaccination activities in the area; Organize retraining for staff participating in vaccination work in the area if necessary to ensure safe vaccination practices according to regulations.
Direct local media units to fully communicate to avoid causing panic among people.
The National Institute of Hygiene and Epidemiology supports and provides technical guidance to localities in investigating and verifying the causes of post-vaccination incidents and proposing corrective actions in the coming time.
The report is requested to be sent to the Department of Preventive Medicine before May 13, 2023 for synthesis.
