Doctors from the Trauma Department of Thanh Hoa General Hospital have just performed an overnight surgery to successfully reattach the severed lower leg of a male patient who had a work accident. This is the fourth case of reattaching a severed limb to be successfully performed at Thanh Hoa General Hospital.

The patient is Mr. LVT, 58 years old, residing in Luong Son commune (Thuong Xuan). While operating a tree cutting machine on an acacia hill, due to carelessness, he was thrown by the machine and severed the lower part of his right leg. Afterwards, Mr. T. was taken to the district hospital for first aid and transferred to Thanh Hoa Provincial General Hospital at 11:00 p.m. on May 18, 2023.
The patient was admitted to the hospital in a state of hemorrhagic shock, lethargic, with a sharp cut on the right leg, severed tibia, fibula, and all blood vessels; exposed muscles and crushed bones, cold right foot, loss of sensation, loss of movement, and undetectable dorsal pulse.
The patient's right leg was given first aid and transferred to Thanh Hoa Provincial General Hospital.
Assessing this as a very complicated injury with a high risk of amputation of the right leg, the surgical team quickly consulted with related specialists. Just 1 hour after entering the hospital, the patient was quickly given all the necessary tests and transferred to the operating room at 0:00. All doctors of the limb reattachment team of the Trauma and Anesthesia and Resuscitation Department were urgently mobilized that night to perform emergency surgery on the patient.
The team of doctors performed blood transfusion, debridement, wound cleaning, surgery to fix the bone with a nail, sutured the anterior tibial artery, posterior tibial artery, great saphenous vein, anterior tibial vein; connected the Achilles tendon, anterior tibial tendon, flexor tendons, extensor tendons of the fingers, connected nerves... using a microscope.
After more than 6 hours of intense surgery overnight, by 6:00 a.m., the team of doctors had completely restored the anatomy, suturing the tendons, blood vessels and nerves of the patient's severed leg.

On the 7th day after surgery, the patient recovered well, blood volume returned to normal, hemodynamics were stable; the surgical wound was dry, the patient's anastomosis recovered well, the right foot was warm and pink with sensation, the toes moved gently and had good circulation, the prognosis was favorable.
On June 2, the doctors continued to perform another skin patch surgery with an area of 10cm2 at the part of the patient's leg that had skin defect. Currently, after 3 weeks of treatment, the patient's leg is recovering well. In the coming time, the doctors will instruct the patient in rehabilitation exercises to ensure that the leg can recover almost its original motor functions.
Dr. Hoang Tuan Long, Deputy Head of the Department of Trauma and Head of the surgical team, said that the area where patient T.'s accident occurred was on a high hill, deserted, with rugged terrain, making it difficult to move the patient down to the ground, so it was not until 6 hours after the accident that the patient was transferred to Thanh Hoa Provincial General Hospital. Therefore, in order to promptly stop the bleeding and take advantage of the golden time to reattach the patient's limb, after receiving notification from the Emergency Center, that same night, the team of doctors from the Department of Trauma and Anesthesia and Resuscitation immediately arrived to perform surgery on the patient.

Severed limbs are serious injuries that not only threaten the patient's life but also severely affect the patient's life and activities later on. The successful implementation and application of the technique of reattaching severed limbs at Thanh Hoa General Hospital has brought about very positive results, giving hope to many cases of limbs that unfortunately were severed during work and daily activities, receiving timely emergency surgery right at the local level, reducing severe sequelae for patients, helping them easily return to normal life.
