"In this woman, due to the old cesarean section from two previous births, the uterus pressed against the bladder. Meanwhile, the uterine tear from the previous cesarean section extended to the right hip, close to the ureter. After removing the baby and suturing the rupture to preserve the uterus, the doctors saw signs of edema that needed to be treated promptly," said Dr. Trang.
Immediately, Dr. Khanh Trang performed a bladder catheterization to examine the bladder and placed specialized instruments to examine the ureter. The results showed that the patient was fortunate to have no damage to the bladder or ureter. After that, the doctor continued to suture the previously ruptured blood vessels to prevent the risk of edema; the mother also had a drainage tube placed into the abdomen for monitoring.
Doctor Trang said that uterine rupture is an obstetric complication that can easily lead to death for both mother and child because every minute the blood vessels rupture can cause a loss of 400-500ml of blood. Thus, it only takes about 10 minutes for all the blood in the body to run out, leading to cardiac arrest, causing the fetus to die first, followed by the mother.
Healthy baby girl after 2 days of birth
In this case, the pregnant woman already had 3 children, including 1 normal birth and 2 cesarean sections. This is an unsafe pregnancy. Normally, if the pregnancy is on an old cesarean section scar, the pregnant woman must have regular prenatal check-ups, and the doctor will admit her to the hospital for monitoring when the fetus is mature enough (37-38 weeks) to be assigned an active cesarean section. However, in this case, the pregnant woman did not have a full prenatal check-up to get advice, and when she was admitted to the hospital, the uterus with the old scar was too much to bear and ruptured.
"It is unclear when the patient's uterus ruptured, but the pregnant woman was lucky to be admitted to the hospital in time. The time of admission was not during rush hour, and there were no objective factors such as the operating room not being jammed... In addition, when the patient was admitted, the team of experienced doctors on duty handled the situation very quickly and promptly to save both mother and child," said Dr. Khanh Trang.
By the morning of June 8, Dr. Hoang Le Minh Hien, Deputy Head of the Social Work Department at Hung Vuong Hospital, said that two days after undergoing emergency surgery, the health of the mother and baby was stable. The baby girl was able to stay with her mother and was fed milk from the hospital's breast milk bank.
According to world statistics, for every 1,000 pregnancies with 1 previous cesarean section, there are 5 cases of uterine rupture, this rate increases 4 times for those who have had 2 pregnancies and cesarean sections. Therefore, pregnant women with a previous cesarean section scar in their uterus must go for a pregnancy check-up and fully report to the doctor for specific advice and guidance.
