While living normally, suddenly the man NHĐ (59 years old, living in Cu Chi district, Ho Chi Minh City) had blood in his urine and was taken to the hospital by his family. There, the doctors discovered that the patient had an advanced stage of kidney tumor.
Taking the medical history, the patient said that he suddenly had bright red blood in his urine, blood clots, pain in his right flank, and urinary retention due to a large amount of blood clots in his bladder. Although he had never experienced this condition before.
At Xuyen A General Hospital (HCMC), doctors from the Department of Urology conducted an examination, combined with paraclinical tests, and recorded that the patient had a right kidney tumor measuring 47x52x72 mm, the tumor had spread to the renal pelvis. Through vascular ultrasound, the doctor discovered that the kidney tumor had buds in the renal vein. The patient was indicated for surgery to remove the tumor. The results of the renal scintigraphy showed that the left kidney function was good, and could ensure kidney function after right nephrectomy.
On November 1, Master - Doctor Nguyen Vinh Binh (Head of Urology Department, Xuyen A General Hospital - Ho Chi Minh City) said that this is a difficult surgery because the tumor is in an advanced stage, with a high risk of bleeding due to having to cut the blood vessels to remove the buds. After consulting and advising the patient's family, the doctors assessed that this case is still possible with laparoscopic surgery.
During the surgery, it was noted that the bud had not spread into the vena cava, so the doctor clamped the renal vein close to the vena cava junction. Then, the doctor cut the renal artery and vein and removed the tumor and the bud through a small incision in the right iliac fossa. At the same time, the doctors performed a cold biopsy of the tumor tissue in the renal pelvis to rule out the possibility that the tumor was an upper urinary tract urothelial tumor, because there were differences in treatment methods.
After 6 hours of surgery, the doctors completely removed the kidney tumor. One day after the surgery, the patient's condition was stable, test results were normal, and the patient could walk and eat normally. Thanks to laparoscopic surgery, the incision was small, not very painful, and the patient's recovery time was faster.
Renal tumors have no symptoms and are difficult to detect.
Specialist 2 Nguyen Vinh Binh said that for the above patient, this is a difficult surgery because the tumor is in an advanced stage, especially with a tumor bud in the vein, with a high risk of blood loss, so during the dissection process, the surgeon needs to be skillful, precise and coordinate smoothly with the members of the surgical team. Kidney tumors are often asymptomatic and are only discovered by chance when you go for a health check-up, or when you go for a check-up for another disease. When kidney tumors show symptoms, they are usually in an advanced stage.
Doctor Vinh Binh recommends that patients who have any unusual problems such as hematuria, lower back pain, palpable abnormal mass in the lower back, or when detecting an abnormal mass in the kidney through imaging tests,... should urgently go to a specialized urology clinic for timely examination, diagnosis and treatment.
