At the time of admission, the tourist was in critical condition. His blood pressure dropped and he had to use two high doses of vasopressors. Severe respiratory failure required endotracheal intubation and invasive mechanical ventilation.
The patient was quickly transferred by A9 emergency doctors on duty to the intervention room of the National Heart Institute for coronary angiography, while receiving maximum resuscitation treatment, ensuring hemodynamics throughout the coronary angiography procedure.

"The results of the coronary angiogram showed that the patient had a complete blockage of the left anterior descending artery (LAD) due to a blood clot. A stent was then placed in the LAD, and after the intervention, the flow was restored well.

In a case of cardiogenic shock after acute myocardial infarction, with multiple organ failure (kidney, liver, respiratory, circulatory), continuous monitoring of hemodynamic parameters and assessment of heart-lung, heart-kidney interactions, and assessment of the patient's oxygen supply-demand balance is very important.
The goal is to ensure adequate cardiac output compared to the patient's needs, minimize the time on mechanical ventilation, reduce the dose of vasopressors, maintain renal function to shorten the time in intensive care due to multiple organ failure, and reduce the need for mechanical support as quickly as possible.
Based on continuously obtained hemodynamic parameters, "SHOCK - Team" of A9 Emergency Center with a team of young doctors and enthusiastic emergency resuscitation residents worked together to solve this difficult problem.
After many days of treatment with great effort, two attempts to remove the endotracheal tube failed because the cardiac contractility function was only 20%.
There were even times when it seemed impossible to continue, but thanks to the patient's perseverance and the utmost efforts of the medical team and nurses at A9 Emergency Center, the patient was completely conscious, breathing oxygen with glasses and receiving intermittent blood filtration to support the stage when the kidneys were still damaged.
The patient was then discharged from the hospital and returned to Malaysia to reunite with his family. Furthermore, the patient's organ functions fully recovered without any sequelae.

After learning about the development trend of Swan - Ganz Catheter - a new treatment method - in the group of cardiogenic shock patients in the world that is returning and there are quite detailed reports published in prestigious medical journals, exactly 2 years ago, a group of hemodynamic doctors from A9 Emergency Center went to Thailand to study, practice with cardiogenic shock experts and apply Swan - Ganz Catheter on cardiogenic shock patients.
Upon returning to the hospital, the team immediately began work on unifying a standard protocol suitable for conditions in Vietnam and strictly following the technical procedures of the Ministry of Health.
By February 19, 2023, the first case had been successfully treated by the team. This was a great encouragement for the team to continue on the path of learning, in-depth research on hemodynamic exploration and answering unanswered questions of complicated cardiogenic shock cases.
Up to now, there have been many cases that have been successfully treated with the support of the Swan-Ganz Catheter, allowing patients to return to normal life.
Currently, A9 Emergency Center is implementing the topic "Research on the role of Swan - Ganz Catheter in hemodynamic resuscitation for patients with cardiogenic shock due to acute myocardial infarction", the topic is assigned to PhD student Pham Xuan Thang to implement under the guidance of Associate Professor, Dr. Nguyen Anh Tuan and Dr. Nguyen Huu Quan.
