Medical news June 20: Warning about perforated viscera due to gastric ulcers with a tendency to get younger
Bai Chay Hospital (Quang Ninh) has just performed emergency surgery for perforated hollow organs on 2 patients under 18 years old with peritonitis due to perforated gastric and duodenal ulcers.
Perforation of hollow viscus due to gastric ulcer is rejuvenating
Patient T.D.K. (15 years old, residing in Gieng Day ward, Ha Long city, Quang Ninh province) was admitted to Bai Chay Hospital for emergency treatment with sudden, severe abdominal pain in the epigastric region and abdominal cramps.
CT scan of the abdomen showing free gas and fluid in the abdomen due to perforation of the pylorus and duodenum. The doctors had a specialist consultation and unanimously diagnosed peritonitis due to perforation of a hollow viscus and indicated emergency laparoscopic surgery to treat the perforation of a hollow viscus.
During the laparoscopic surgery, the patient had the perforated hole in the anterior part of the duodenum sutured, the dirty fluid was cleaned with 5 liters of isotonic saline, and the abdominal cavity was drained.
Another case is patient BTQ (17 years old, residing in Le Loi ward, Ha Long city, Quang Ninh province) with a history of gastric ulcer surgery 2 years ago. The patient had signs of severe pain in the epigastric region, abdominal cramps. The abdominal CT scan results showed a lot of fluid, free gas in the abdomen due to perforation of the gastric pyloric antrum.

The doctors consulted with specialists to diagnose peritonitis due to perforated hollow viscera on the basis of a patient with a history of gastric perforation surgery, performed emergency laparoscopic surgery to suture the perforated pyloric antrum, cleaned out the dirty fluid, and drained the abdominal cavity. Currently, the health of the patients after surgery is stable.
Dr. Chu Manh Tuong, Department of General Surgery, Bai Chay Hospital said: There are many causes of perforated hollow organs such as: perforated stomach, abdominal trauma, foreign bodies in the digestive tract, ruptured tumors, necrotizing enterocolitis... the most common complication is perforated gastric - duodenal ulcer.
Perforated hollow organ is a dangerous surgical emergency. If not diagnosed and treated early, it will lead to life-threatening complications because food, digestive fluids, and blood can spill into the abdominal cavity, causing abdominal infection (peritonitis), infection, abdominal poisoning, etc., even death due to systemic septic shock.
In perforation of hollow organs, it is more common in men than in women. The disease is more common in men aged 35 - 65, but most common in men aged 30 - 40. If surgery is performed late, the mortality rate is from 2.5 - 10%, in elderly patients the mortality rate is higher. Currently, perforation of hollow organs is tending to be younger in age due to stress, pressure from studying and working, the habit of staying up late, unscientific eating (eating at irregular times, eating a lot of sour, spicy, hot foods, fast food...) causing ulcers in the stomach - duodenum, complications leading to perforation of hollow organs are very dangerous.
For the pathology of hollow visceral perforation due to gastric and duodenal ulcers, doctors at Bai Chay Hospital recommend: When there are signs of epigastric pain, belching, heartburn... patients need to go to a medical facility for examination to detect early gastritis, gastric and duodenal ulcers, to treat and avoid dangerous complications such as ulcer perforation, gastric bleeding, cancer...
Young people need to adjust their lifestyle, study and work in moderation, build a scientific and punctual diet and rest schedule, limit eating late, staying up late, and have regular health check-ups to promptly detect and treat diseases related to gastroduodenal ulcers.
Cooperation in screening, clinical nutrition intervention, improving treatment quality
The National Children's Hospital and the Vietnam Pediatric Association have just collaborated with Abbott Vietnam to organize a cooperation program "Clinical nutrition screening and intervention, improving treatment quality (QIP)".
The cooperation program aims to improve the quality of treatment for pediatric patients by combining medical treatment and nutritional care to help children increase their ability to recover.
The main activities of the cooperation program include:
Screen and assess nutritional status for all pediatric patients who come in for examination and admission, as part of the patient's medical record.
Enteral nutrition intervention for children at nutritional risk or malnourished as appropriate to the clinical condition, as part of treatment.
Nutrition education programs are designed for specific medical/health problems (such as chronic illness, pediatric surgery, hospitalized child nutrition).
Training programs to improve clinical nutrition capacity for health workers.
Reports show that in addition to medical treatment, nutritional care for patients plays a very important role.
Not ensuring nutrition will cause children to respond poorly to treatment, increase mortality rates, slow recovery, prolong treatment time and costs, affect children's development, etc.
However, this issue has not received due attention in many medical facilities. Therefore, to improve the quality of treatment, screening and assessing nutritional status for inpatients and outpatients is essential.
In 2019, the National Children's Hospital built a nutrition network with the participation of doctors and nurses in clinical departments. By using pediatric patient management software with simple operations, doctors and nurses can fully assess the nutritional status of children from admission to discharge. Thereby, helping children recover faster, reducing treatment time.
Associate Professor, Dr. Tran Minh Dien - Director of the National Children's Hospital, Chairman of the Vietnam Pediatric Association expressed his joy and appreciation for Abbott's cooperation with the Hospital and the Vietnam Pediatric Association in the effort to take care of children's health.
Through this cooperation program, the parties will develop strategic plans in the coming time to best assess the nutritional status of children coming for examination and treatment.
In addition, these assessment processes will support lower-level colleagues, thereby helping to detect children at risk of malnutrition in the hospital early and have timely interventions.
Within the framework of the program, delegates also discussed and gave opinions to unify the screening toolkit for assessing nutritional status in inpatient and outpatient treatment to be applied in practice in the coming time.
