Ministry of Health guides plans for rearranging and reorganizing grassroots health facilities

The Ministry of Health has just sent an official dispatch to the People's Committees of provinces and centrally-run cities on guidelines for the arrangement and reorganization of medical facilities at administrative units at all levels, implementing the 2-level local government organization model.

Reorganize and reorganize medical facilities at district, county and town levels

Accordingly, for the Health Department under the People's Committees at district, county and town levels, according to the guidance of the Ministry of Health, it will be dissolved, end its operations, and transfer the functions and tasks of advising on state management of the former Health Department to the Department of Culture - Society and the Public Administration Center under the People's Committees at the grassroots level (commune and ward level) after being arranged to continue implementation.

The tasks and powers of the People's Committee at the grassroots level (commune and ward level) in state management of areas under the state management of the health sector in the commune and ward are implemented according to the Appendix attached to this Official Dispatch.

Directors of the Department of Health of provinces and cities are responsible for supporting human resources, directing and providing professional guidance to assist the People's Committees at the grassroots level (commune and ward levels) in performing the tasks and powers of state management in areas under the state management of the health sector in the communes and wards.

For existing district, county, town and city general hospitals: Basically maintain existing district, county, town and city general hospitals and specialize into regional general hospitals, which are public service units directly under the Department of Health of the province or city after the arrangement, to organize the provision of medical examination and treatment services for people in inter-commune and ward areas regardless of administrative boundaries.

For District, County, Town and City Health Centers: Basically maintain the existing District, County, Town and City Health Centers and convert these Health Centers into Regional Health Centers, which are public service units under the Department of Health of the province or city after the arrangement, to perform the functions, tasks and powers according to Circular No. 32/2024/TT-BYT dated November 15, 2024 of the Ministry of Health guiding the functions, tasks, powers and organizational structure of District, County, Town and City Health Centers under provinces, cities and cities under centrally run cities and continue to support human resources, professional and technical guidance, ensure medicines, supplies and medical equipment for commune health stations of communes and wards after the arrangement.

Based on the review and assessment of the scope of operations and operational efficiency of existing district, town, and city health centers, the Director of the provincial and municipal health department shall advise and develop a project and submit it to the People's Committee of the province or city to decide to maintain all or dissolve, reorganize, or merge a number of district, town, and city health centers in the area.

Each new commune and ward health station has at least 2 doctors or more.

Maintain and preserve existing regional polyclinics, commune, ward and town health stations and transfer them to the People's Committees at the grassroots level (new communes and wards) for management. Ensure that each new commune and ward after the arrangement has at least 01 health station; Depending on the population size and geographical characteristics of each new commune and ward, maintain or reorganize and establish additional health stations managed by the People's Committees at the grassroots level (commune and ward level) to meet the primary health care needs of people in that commune and ward.

The functions, tasks and powers of the Commune and Ward Health Stations continue to be implemented according to the provisions of Circular No. 33/2015/TT-BYT dated October 27, 2015 of the Ministry of Health guiding the functions and tasks of the Commune, Ward and Town Health Stations.

The human resources of the new Commune and Ward Health Stations are arranged on the basis of receiving officials from the previous Commune and Ward Health Stations and are mobilized and supplemented from provincial-level medical facilities, from the previous District, County, Town and City Health Centers to ensure that each new Commune and Ward Health Station has at least 2 doctors or more.

The Ministry of Health said it will issue a Circular guiding the functions, tasks, powers and human resource structure of Commune and Ward Health Stations to replace Circular No. 33/2015/TT-BYT dated October 27, 2025 of the Ministry of Health guiding the functions and tasks of Commune, Ward and Town Health Stations.

The Ministry of Health also stated: Directors of the Department of Health of provinces and cities are responsible for directing organizations and units under and directly under the Department of Health to support grassroots health stations (commune and ward levels) in terms of human resources, professional guidance, ensuring adequate medicine, equipment, medical supplies and maintaining health insurance medical examination and treatment contracts at health stations to ensure the efficiency, effectiveness and efficiency of the operations of grassroots health stations (commune and ward levels), without interruption in the provision of health protection and care services to people in the new communes and wards.

The Ministry of Health recommends that the People's Committees of provinces and cities base on these instructions and local practical conditions to implement, ensuring the effectiveness of the operations of medical facilities after rearrangement and reorganization.

In the process of arranging and reorganizing medical facilities in the area, if there are any difficulties or problems, the People's Committees of provinces and cities will discuss and coordinate with the Ministry of Health to continue to guide implementation in accordance with regulations.
